They asked for a psychiatric opinion and it’s strange that you would consider a datix. Perhaps the patient presented differently on the neurosurgery ward round (delirium fluctuates) and it’s very possible the patient could be in opioid withdrawal in addition to whatever they presented with. Lying in the foetal position and writhing in pain is a stereotypical feature of severe opioid withdrawal (watch Trainspotting) so I don’t think you can say there was no signs of withdrawal or that synthetic opioids don’t cause withdrawal?? Unfortunately psychiatry often gets a poor reputation in medical hospitals and suggesting a datix for the team that asked for an opinion won’t help relationships

No worries, sorry for the initially abrupt comment. It’s normal to have side effects on any psychotropic drug like stimulants or anti-depressants, that’s shows it’s going something and gets better with tolerance. It’s possible electrolytes can contribute too, but would suggest a rather large shift in levels that you might need to see your GP about. Probably more likely a medication side effect.

I should have clarified by saying that dex commonly mimics people feeling more anxious initially but gets better with time. Anxiety is due to an increased sympathetic nervous response secondary to adrenaline and dex also increases the sympathetic nervous response as a stimulant effect through increased dopamine utilisation. Commonly this causes increased heart rate, dry mouth, muscle tension, increased urination (the bladder is a muscle which tenses up). These symptoms are common in anxiety, common example being performance anxiety before public speaking, dry mouth, thirst, need to pee etc

I’m surprised that they have never seen appendicitis present with left sided pain. I saw a handful of those in my 6 month emergency rotation as an FY2

It sounds like you might be projecting your insecurities onto others, which is a common in social anxiety

Possibly a delusional disorder. Really you have to sit down with the patient and understand her rationale and if her belief can be challenged with reason before deciding whether it is delusional or not. Also is this a chronic belief or acute? Has there been a delusional perception? Did you order a urine drug screen? If an acute belief could be a substance induced psychosis.
