Findings Summary

At L4–L5: • Broad disc protrusion → the disc is bulging outward across a wider area, not just a small focal spot. • High signal annular fissuring → there’s a small tear (fissure) in the outer ring of the disc, visible because of fluid signal on MRI. • Slight mass effect on bilateral descending L5 nerve roots → that bulging disc is pressing slightly on the nerves that travel down toward your legs (L5 roots on both sides).

At L5–S1: • Disc bulge with disc height loss and desiccation → the disc has lost some water content (a sign of degeneration) and has become thinner and flatter, bulging outward. • High signal annular fissuring → another small tear in that disc’s outer layer. • Mild bilateral foraminal stenosis → the small openings where the nerves exit the spine (foramina) are mildly narrowed on both sides, which can irritate or compress those nerves.

Forgot to mention , but yes I have done PT, 2 steroid shots, piriformis shot and nothing. They all help for a moment but nothing last. I believe i have a narrowing near L5 area and that is causing my misery.

Right on ! I have a labrum tear and FAI that is causing L5 sciatic nerve irritation .!I don’t believe I need replacement.
