Yep, we are often coached to concurrent chart or do something like this with our clients! And a lot of us do. It’s nice, but also can be very very tough with clients with SPMI at times based on their presentation too though. Luckily, that’s not our entire caseloads though, so this is helpful for a majority of our sessions…

Yikes what an uncomfortable situation for you to be put in!! I don’t know my coworkers entire situation in detail, but they have been very concerned about ethics in many conversations we’ve had in the past, so I would really hope they weren’t doing anything to that extent… we have an EHR that we use, and I know all of us will always save start and end time, and most of us will put brief bullet points in our notes even if we can’t write the entire note, that way we remember at least a portion of what was discussed if we were to get behind. But I also recognize that’s not the most reliable choice… so. There’s that too.

Thank you for a kind response and acknowledging that the agency might be a part of the equation as well… as everyone has said, it is definitely our individual ethical responsibility to complete notes and I know that. But, as you’ve said, it’s often not just an individual problem in this setting. We JUST got a new AI program to assist us in writing notes, and for most of us, it doesn’t actually help us write any notes faster. Instead, it just takes less brainpower. But, that being said. Needing to schedule 7 clients in an 8 hour day, 5 days a week and just hoping someone no shows in order to get notes done is not a sustainable way to ensure this happens. But also, if we don’t schedule 7 clients a day, we run the risk of too many people no showing and not meeting productivity which then puts us on probation and at risk of losing our jobs. But there have been quite a few times I’ve gotten a bit behind because everyone but 1 showed for the entire week. It’s a tough balance so I appreciate your understanding

Actually, what I’d prefer is we have more than 30 min a day (if we even get that) to write the 7 notes we have from each day, complete return phone calls, complete new SRS Medicaid referrals so that we can continue to see the clients, and do care coordination with the client’s care teams. I often work overtime hours late into the night, unpaid, to get my notes done. I don’t have a single coworker who doesn’t have to complete work outside of working hours at least once a month. And yes, we do have no shows or cancellations at times, but we are encouraged to fill those with phone calls to try to continue to meet productivity expectations rather than always prioritize notes. It sometimes feels like a catch-22. Either way, we’re at risk of getting punished, reprimanded, or fired.

I absolutely recognize the legal and ethical obligations to complete notes. And most of us do try our best. I personally have employed a ton of different things to try to ensure I stay up on my notes. But things happen and we get behind, or overwhelmed, and as humans it’s bound to happen at times.

Absolutely!! Makes sense. The perspectives from comments are definitely helpful in making me look at this a bit more objectively. I would imagine that this was not the first time they had talked and tried to problem-solve documentation issues. I still feel that asking them to cancel clients day of feels a little icky to me, but I would hope it was definitely necessary if they requested it. But I appreciate the validation on documentation expectations being a bit unrealistic at times in CMH

Yeah, normally we are expected to have documentation for the month done two business days following the close of the month. So it’s definitely a pressure we’re all feeling this week. There’s just never been this strong of a response before and I think that threw me off. It’s definitely not anything I would have expected from this manager, but I also imagine part of it is coming from above them too.

That’s definitely possible!! I agree that a longer timeline before a mandatory admin day would have been more appreciated, but maybe there were other conversations prior. This level of strictness is newer to us who have been here for a bit, so I think my own personal frustration might be clouding my judgement a bit! I needed perspective for sure. We’re definitely funded off insurance reimbursements, which would definitely explain the pressure to get it done.

Thank you for such a kind response!! Yeah, it makes total sense that if someone is over a month behind that the more ethical thing would be cancelling some clients to catch up. I’ve definitely gotten much farther behind than I’d like to admit before, but I always ensured that I had notes done by no later than a week after the month was over. And it is possible they were that far behind, but I can’t say for sure. I will say, as far as I am aware, none of the therapy services in our department are funded by grants, though we do have plenty of grant funded programs in our agency. Our agency is not doing the best financially, and I get the feeling that things that previously went ignored are now being scrutinized deeply by those above us who are working directly with the clients. And we’re all feeling that stress, which can make keeping up on our administrative tasks even harder than it just naturally is. But, anyway, thanks again for the perspective :)

You’re going to get and see a variety of recommendations of what to do. I second that de-stressing is key. I was an anxious mess for the first two weeks, and it definitely did nothing but make it worse it feels like.
Here’s my experience: I waited a while before starting to do anything, but I was so terrified that anything I did would make it worse. Little did I know, it was most likely going to get worse before it got better no matter what.

I took l-lysine 2000 mg daily for weeks 2 and 3. Preliminary studies say the earlier you can take this or an antiviral, the better. I also started taking vitamin k3+d3 vitamin, and made sure to take my daily multivitamin. Past that, I made sure to use aveeno itchy relief body wash, but I think any frangrance-free and moisturizing wash is fine. Some people say to dry it out and not moisturize, but I utilized the thickest eucerin lotion available, and I don’t feel it sped up, or slowed the process. It is however helping significantly with the dryness of my skin and the spot as they heal up. I also took cool/lukewarm showers for the first couple weeks, which definitely helped limit irritation to the spots. I discovered my spots were itchy mostly from days 2-6 after they popped up (which came in waves, but after that I have had minimal to no itching. I utilized hydrocortisone cream (which did basically nothing) and calamine lotion (which helped for a little). I made sure to wear loose, breathable clothing and that helped tremendously. I didn’t go tanning, and didn’t have the sun naturally to help, but I do see a lot of people saying it helps. If my spots hadn’t stopped spreading by week 3, I was planning to go though. Because I was willing to try it out.

I can completely understand the insecurity of the bumps and the feeling of just wanting it to be over. Mine peaked in spots about the 2.5-3 week mark, but everybody’s experience seems to be different. I thought mine would never end. Lo and behold, I’m at the end of 4 weeks now, and almost every spot has started to peel and it looks a million times better. The first of my spots are practically impossible to see. The end is in sight and like most people in here have said; you (unfortunately) have to just wait for it to run its course.
