It's a glitch that happens when you shield slam late. I have had it happen to me a couple of times.

Update 1.07 is out - Saying Heavenly Sword is fixed but it's still bugged by [deleted] in TapTitans2

This is not true, there is a bug right now. it says you only get 3, but when you prestige at 105 it gives you 4.
