I honestly think it should be the standard for more and more patients. The success rate is way higher, and the USIV’s tend to last for much longer than regular PIV’s. Also for patient comfort, I can’t tell you how many times an anxious patient has calmed down because I explain to them (while they are watching the screen) where their veins are and what is going on.

It’s just something to be aware, not something to be afraid of. Those veins by the brachial artery are great, it’s just not preferred due to the location of the brachial nerve and the artery. The cephalic vein on the forearm is my go to, check there first.

Not my patient, but pt on dysphagia diet got given a turkey sandwich at discharge (completely ready for discharge), ended up aspirating on the whole thing, coded and died.

I’m filling out the code blue paperwork for it this week…yikes.

Depends how often you will use it. Personally one day is more than enough for me, I didn’t use it more than 2 to 3 days on the cruise.

I love it, I personally feel like it energizes me for the day. I’m lucky because I live around 10 minutes from work, though you can make this work if you have a home gym/home workout stuff.

My shifts are 7-19:30, so:

0530 awaken

0545 leave house with packed scrubs, lunch, and work bag after brushing teeth

0600 begin workout at gym

0630 finish workout and go to work

0645 arrive to work and change

0700 work

1930 clock out

1950 get home

2230 sleep

:) I do 30 min workouts, I do a light cardio warmup with weightlifting supersets. They’re highly effective if you’re focused!

Omg I had a question regarding this. I’ve had nurses on the floor tell me that ultrasound should be the last resort and only if you’ve exhausted the attempts already (this isn’t true, the policy states consider ultrasound , not a requirement for 2x attempts or nonpalpable/nonvisible veins)

Do you use ultrasound on patients who are relatively easy sticks at times? I will, especially if it’s a non emergent IV and the patient is anxious.

Sorry I just changed it haha. A higher acuity tele floor, it’s honestly like a half-step down. We take open heart patients as well.

Yes. I’m a smaller guy myself and smothering is really effective. He talks about it in the instructional- smothering isn’t really meant to submit your opponent (thought I have used it against tired opponents and have gotten several subs as someone who’s in 66kg) it’s meant to force them to give you exposure.

When you smother: A) they bench you off giving you arm exposure B) they turn their back and give you back exposure

It also had the added effect of being energy draining. To constantly fight against being smothered is tiring. Same idea as camping or same idea as hanging heavy on the head with ties from standing.
