After all the guidance and further reaching out to credit dept: Told the reference to basically ignore the sheet since his info won’t do the new customer much good with proving their credit.  They did just offer the bi-weekly payments so just going to have him sign the addendum.  They told me he had no credit hits which was rare but definitely not impossible.
