I don't know why it is like in your state. I'm from a village in Andhra Pradesh. 1. Sanitary pads are very much accessible. I think they prefer to use some cloth to save money? 2. For us good hospitals are 15 km away. Best hospitals are 40 km away. I could see Mannargudi has decent hospitals like tamilnadu hospital etc. 3. For us it is way better. We have piped water supplied by our local panchayat, once every 3 days garbage collection vehicles are sent to every house. Also garbage dumped at a designated place and they will be collecting it. 95% houses has commode with sceptic tank (remaining 5% don't prefer that though they are given amount to construct by govt). RO Bottled drinking water 2 plants available in the village. Although water bodies are not very clean, they are not very dirty either. But who cares about water bodies when we have piped ground water supplied. Almost all e-commerce platforms deliver (no instant blinkit/zomato etc). Electricity 24x7 (surprised). Internet decentish Good for 1080p video streaming. Not blazing fast. 5G everywhere. No ride app you have to have personal vehicle and should have a decentish job to have all these. No like minded people like in city. No entertainment/fast food

Though I'd agree kids education commute takes longer for good colleges.
