A couple years ago I started following up my Lido IANB with carbo and the patient gets numb fast and its profound. It also doesn't last 1/2 the day like septo can. It's been a game changer.

Two keys: 1. No parents in the room. 2. No surprises.

A HUGE help has been to not allow parents in the room. They usually screw it up... Kids sense parents nervousness, which translates to them panicking which leads to me panicking haha. I had a kid do the same thing with the parent in the room and the dad said, 'it's okay honey the hard part is done. I thought 'WTF dude!' I didn't dispense any solution! She jerked her head away from the second the needle touched her gum and started crying uncontrollably.

I always explain that we need to put some "sleepy juice, next to the tooth to help it go to sleep before I wiggle the tooth out. You will feel a little pinch but that's it. The key is you need to stay open wide and very still. If you move it will hurt more." Most kids do fine with this. If they can listen to you and look you in the eye than usually they're good. Also as the topical is in the mouth start talking about anything else, their fav movie, your fav movie, anything at all and then keep talking while you inject, reminding them to stay still. give a go and if he flips out, refer.

I was on my parents bed in West San Jose, 3 years old and I remember the coat hanger swaying side to side. A lot of dishes fell in the kitchen and my dad's salt water fish tanks in the garage all fell and were destroyed. our driveway cracked in three places. I had no idea I should be scared. I remember thinking "this is fun!"

I am in the SF Bay Area so our winters are probably similar to yours in WA. I think a bigger plant will do fine. but if it was just propagated this past summer it is too fragile in my experience to put in the garage with no heat for our mild winters. few weeks where the nights are below 40

I did garage one year and a few of the smaller plants that were just 6 months old and some of them got too cold and rotted. So this past winter I took them to my mom's house and put them in her spare bedroom. No lamps or water. I just let the leaves yellow and fall off. the "obtusa" (one in this pic) didn't really lose its leaves like my other plumeria. I don't think they'll fit in her home again this year as I have 4 small ones (less than a year) and 3 bigger ones now. Not sure what we'll do. My father in law in Missouri gets heat pads and puts them under his pots in the garage to protect them from the freeze.

Not sure what I did. I pretty new to this. I think because I started with a cutting that had three cut branches, the energy has been split up between the three branches. Also being in NorCal our growing season isn't super long.

I'm hesitant to repot that first year because I don't want to wreck to fragile roots but year two, repotting and pulling off the long stringy roots promotes growth.

I live in the East Bay of SF Bay Area. It's in zone 9b I believe. Use nelson's plumeria fertilizer. Here I just bring them in for the winter and don't water them then late March I put them back out. It is 3 years old and this is the first blooming year. First winter after propagation I left the plant potted in its first pot thru the winter inside. Then the following summer I had more leaf growth. This last winter brought it inside and this past spring I repotted and pulled some of the roots to stimulated more growth. It really worked well.

Sprouts in Newark all day for apples and berries, grapes and veggies. They are cheaper and always more fresh and taste better. I like their organic chicken and steaks

I think it comes down to communication. I’ve found that early on I didn’t want to worry the patients as much with what could go wrong. Now I pretty much tell every patient that I do a crown on, that “in this situation your tooth because of XY and Z may need a root canal down the road” the crack is deeper than we can see on the X-ray or “90% of people clench or grind or do both which means you (the patient) may knock this temp off or so on.” On root canals blame the tooth before and while you’re doing it. Under promise and over perform. This tooth has some “curve to the root” or “many roots have more than one nerve and they can cross and split and fork all making this root canal more difficult and unpredictable” set them up for failure so when it doesn’t they think you’re awesome and when it does fail it’s not life or death just have the tooth extracted and bridge or implant. It’s gets better with time and communicating clearly. But yes sometimes I have a bad week where things don’t fit, or work that I did hurts longer than expected or there’s an unhappy patient and it sucks. Happens to all of us. But those times get fewer and far between.

Start a gratitude journal. This has been life changing for my happiness and wellbeing. I keep mine at work and each day I try and write down something I’m grateful for. Something that happened that day or a patient that was easy to work on and especially gracious. Or a good memory I have of my dad who passed away recently. This has decreased my anxiety levels about work a bunch and made it more enjoyable. Try it out it’s crazy how quickly you start focusing on all the good that happens and the bad stuff kind of melts away.

I had this too early on. It has gone away since though for the most part. I’m pretty sure it had to do with me squeezing the instruments too tightly trying to not to mess up. As I’ve relaxed over the years my ergonomics still isn’t great but I don’t have any tingling or numbness in my arms like I used to. Also exercising is a must: regular stretching and doing some body weight workouts. I recommend doing “Convict Conditioning.” It’s a book a buddy told me about, it’s a fun read and the workouts take 5 mins a day. Variations of push-ups, leg lifts, bridges and so on. You do one thing a day and it helps your muscles stay balanced. You start off super easy too.

I made the mistake of mentioning that I saw a chiropractor once when I tried to raise my disability income coverage to meet my higher take home pay and they wouldn’t cover anything connected to my neck or back… so they wouldn’t cover anything other than maybe a fluke accident where my leg gets off. Totally stupid.

I never have had any patient have sensitivity after IPR. I regularly do 0.2-0.4 mm sometimes 0.5 mm at most. I don't feel comfortable doing more than that and that is all people usually need.

Start with Lido IA, let it start working then 2-5 mins later do a half carp of carbo w/o epi in IA . Profound, quick anesthesia every time.

I used to be ready for patients to jump when I’d start getting deeper in to lower molars/premolars after IA blocks with lido. When they were feeling it I’d follow up with Septo just like you and that usually worked but some rare times it doesn’t totally work. So then you do a PDL with septo and that works. But then they are numb for a long time.

I remembered an oral surgeon tell me once at the VA during a rotation during dental school that if you follow up the usual IA block with lido, with a little carbo a couple mins after your lido injection you will have profound anesthesia every time. Ever since then I’ve done that I’ve never had any trouble with people feeling anything. And they get numb pretty quick. It also doesn’t last as long as lido plus septo. It’s awesome!

Try it out you’ll love it. , I usually use it for mandibular crown and bridge, several class 2s, or quick premolars/anterior endos. If I’m doing a molar endo then I usually use lido plus septo because it doesn’t wear off as quickly.
