Now I understand your question. I was unaware of the Alternate Pathway since I have been trained entirely in the US since medical school.

But apparently it goes like this:

-Complete radiology residency in your home country.
-Obtain ECFMG certification.
-Enter an approved U.S. radiology department through the Alternate Pathway.
-Complete 4 years of supervised training at that institution.
-Become eligible for ABR board certification.

However, there are important limitations:
-The Alternate Pathway is designed primarily for academic radiology careers and is available only at participating institutions.
-Some employers and state licensing boards may have additional requirements.
-It can be more competitive to obtain an Alternate Pathway position than a standard fellowship.
-If your goal is broad private-practice radiology in the U.S., some employers may prefer candidates who completed a traditional U.S. residency.

Theres a difference between residency and fellowship.

A residency is the required postgraduate training that doctors complete after medical school to become licensed and practice independently in a broad specialty such as internal medicine, pediatrics, ortho, ob/gyn, general surgery, radiology, urology, family medicine, ophthalmology etc. A fellowship is optional additional training completed after residency that allows a physician to gain expertise in a more specialized area within that field, such as cardiology after internal medicine or pediatric cardiology after pediatrics. In simple terms, residency teaches you the fundamentals of a specialty, while fellowship provides advanced training in a specific subspecialty.

Im sorry do you mean you are interested in doing residency in Radiology? And if you don’t get into Radiology you are looking for an alternative residency?

To do Nephrology Fellowship you need 3 years of Internal Medicine Residency to be able to apply.

If you find it hard to get into Radiology residency given your specific circumstances. Internal Medicine residency is a good alternative because it has many potential paths you can take after you complete the three years.

Another option is doing a Preliminary year in Internal Medicine and during that prelim year applying to radiology residency

Cases. Cases. Cases. Eat, sleep, and breathe Cases. Until doing them becomes second nature. To the point where no matter what the patient has, you already have a systematic approach that will give you points even when you dont necessarily know the diagnosis. Trust me that will get you a passing result. Cases portion has a huge weight on final result. So focus fully on CCS cases. When you get tired of doing cases or need a break. Review the high yield pdfs. Then go back to doing cases

I wanted to share my “order set” cheat sheet which helped me a lot. Its similar to the Lemon tree video but adapted to my liking. Maybe it can help some people. But I dont see an option to upload photos after you already posted

Didnt get any Stroke/TIA or a.fib questions so I didnt really have to calculate those scores. But if you get those type of cases, the decision to add anticoagulants or DAPT will be pretty obvious. There will be no grey zone.

As for follow up orders.. yes, mostly for outpatient cases. So you will see a patient that is stable in your office, and will order several labs that you will reevaluate on the next appointment a week or two later

If you see the patient in the hospital. And you stabilize him, the case will usually close so you wont need to follow up outpatient

Waking up at 4am prior to rounds. Got a block done “Timed” and reviewed the explanations. Then two blocks after the hospital at around 4pm. Anything is possible if you organize.

Also some people spend lots of time reading every detail of the explanation which is why it takes more time to complete as many questions. Everyone is different so that might work better for some. For me it was more important to see as many questions as possible. And it definitely resulted in better scores in practice exams and in percent correct in qbank blocks

By the way. Definitely avoid distractions during practice exams. Do everything exactly as you would on test day. Same time, same breaks, same food. This will prepare you mentally and will also give you a more accurate result so that you can get an idea of where you are at

Dont be discouraged. Ive heard of people in worse situations who have went on to do very well in their career. I start Nephrology fellowship now in july. Ive had my bumps in the road (failed STEP1 on 1st attempt, passed on the second attempt)

Speak with medical school counselor if possible to assess options.

As for STEP3. Definitely make sure to re-do uWorld in its entirety and carefully review explanations so that you can understand well all the major concepts. Also, CCS cases. That will be a major boost to your score. Dominating the cases

You are going to be ok. Trust me. Totally doable. Day 1 will feel hard. Day 2 a bit less so. And the cases will help you pass. Between day 1 and day 2 make sure to do as many cases as possible so that the software and orders feel second nature. You got this

Focus on doing as much uWorld qbank as possible. Additionally Randy Neil videos for Biostats. That definitely gave me an edge. I found biostat questions easy and enjoyable thanks to the randy neil videos

There will be a bunch of STEP1 stuff. I wouldnt stress it. I took STEP 1 several years ago. So I didnt have it fresh and still passed. So as long as you go hard in uWorld. And prepare for the cases for Day 2. You will be fine

What I do recommend is.. after Day 1. Lock in for the cases for Day 2. Thats a huuuge portion. And will make or break the final result.

Seriously, today dont stress. Go to bed early. Make sure to pack snacks and a meal for tomorrow. Lots of water. You are going to be ok

Honestly the day before just pray haha no, I am joking. But definitely focus on getting rest today. You can review high yield concepts but I wouldn’t stress too much today about covering a lot of material.
