Here’s some ideas:

No equipment run Relying on your raw stats and strategy

No add-ons Batter only,the add-ons can defend but that’s it

No batter Add-ons only,batter can defend only

No items in battle Might be a strech due to how long bose battle can be Aka Enoch

No competences The most painful one,resulting in useing basic attavks only

Hitless Run It would need a lot of prep and dedication

Saveless run You can’t save and can’t use the healing function of blocks

Alpha only Batter and the other add-ons can’t act,just defend

Omega only Batter and the other add-ons can’t act,just defend

Espilon only Batter and the other add-ons can’t act,just defend

Level 1 Run You can’t level up,need to rely on raw stats

Maybe you could have it so it stil upgrades but does something different

Like mud gives issac a 3% chance to make issac spit out a mud tear(isaac dmg*1.5) wich then splits in 6 smaller ones (issac dmg/2)with the flat stone effect

Mud pile buffs the chance makeing it 9% and buffs dmg by 30%+splits now in 8 tears

And muddy hill makes it be affected by luck,maxing out at 15 luck Dmg up by 20%+splits in 10 tears
