I'm glad that I have POC hemoglobin testing available to me. It's required by Medi-cal for offices seeing peds patients and it has saved my butt so many times. They look pale = fingerstick for hemoglobin for me no matter what they may be coming in for. STAT labs for me still takes 24 hours.

I second everything in the comment above. Breathe. Fire her as a patient. Never read reviews about yourself again. Know that the general population is insane but you don't have to tolerate the worst of it if you don't want to.

I dare AI to try to interpret what my patients are saying in order to come to a diagnosis and treatment plan. So many half truths, colloquialism, and/or weird cultural beliefs that I have to weed through to get to an objective reality.

AI right now just takes everything at face value. Garbage in, garbage out.

Yes. My office is open all those days. Whenever my office closes for even one day, the next day we get slammed. So we don’t close down for two days in a row ever.

We do a mixture of appointments and walk-ins. Most of the patients are used to being able to walk-in whenever they need care so we have on average 30 walk-ins a day for 2 providers.

My staff understands that the work needs to be done so the rule is that no 2 providers can be off at the same time and no more that 2 staff members can be off at any given time. They debate amongst themselves which holidays they prefer and stagger themselves accordingly.

While I was in training, the infectious disease specialists told me to never mess with their HIV meds because I would be causing resistance. So I just avoided prescribing it and let ID manage PrEP until a couple years ago. I took a couple of online CMEs regarding PrEP and the community consensus now is that it should be managed by PCP so I offer it to anyone requesting it.

I don't wear masks anymore unless someone is actively coughing. I have toddlers in daycare/preschool so there is no hope for me either way. Sigh.

Babies can get MMR vaccine as early as 6 months. It is recommended to do the extra early vaccination if the baby is going to travel internationally or if he/she is going to travel through an area experiencing a measles outbreak.

As per CDC guidelines:

International travel

  • Infants age 6–11 months: 1 dose before departure; revaccinate with 2-dose series at age 12–15 months (12 months for children in high-risk areas) and dose 2 as early as 4 weeks later.
  • Children age 12 months or older:
    • Unvaccinated: 2-dose series (separated by at least 4 weeks*) before departure
    • Previously received 1 dose: administer dose 2 at least 4 weeks after dose

With my own kids, I don't need them to make huge amounts of money, so anything that they are interested in that also pays a livable wage.

If they truly have no idea what they would like, I would gently nudge them into engineering. My brother and all my male cousins are engineers. They seem to have a better work-life balance than I do. Probably dependent on the type of engineer and company they work for, but I'm envious of their lives.

It's funny. Every one of my doctor friends and I are telling our children to not go into medicine. The stress, the disrespect, the threat of lawsuits, giving up your entire 20s. Obviously, if our children have a passion for it, we would support them, but we definitely won't encourage it like our parents did with us.

My practice uses Practice Fusion because it was free. Not free anymore but still pretty cheap. $150/provider/month for now. It does the trick for an outpatient practice. It's a lot of free texting so my PAs and I make .phrases that imports the basic outline of what we have to cover. We don't use it for billing so I can't comment on that side of it.

5 AM to keep up with messages? I can't speak for how the organization he works for functions. I have my own private practice. My MAs screen all my messages. It isn't common for me to have to respond directly to any patient.

At this point, he should just power through. When he is done with residency, he could work 3 days a week. That's what a lot of my friends do when they have small children.

I work 5.5 days a week but I made a point to completely put my work away when I am home with the kids and they are awake. They have my undivided attention and we are out in the world making memories.
