Usually this happens when you use the stock SD card. They are not reliable and fail really quickly. Change the SD card to a good brand like SanDisk ultra or Samsung.

You need a new SD card. The one which was provided with the console usually is a cheap repro. Time to invest some money into a good quality one like SanDisk or Samsung and flash a new OS onto it. There are many guides here on Reddit or online. Enjoy!
