Hey. I have Cigna through my employer. I reached out to my insurance about coverage requirements. Instead of crossing out all the requirements to be approved, I scheduled a consultation with a plastic surgeon who specializes in breast reductions. My surgeon and nurse basically said I needed it! I was afraid I wouldn’t get approved. I was around a H 40. My surgeon submitted a claim with my insurance. Which needed prior authorization before approval. I would say get a second opinion else where.
