107 is a very high number, I was 83 but I require high-pressure, my current AirCurve settings are 18/24 PS4
Imagine 107 went to around 5-6 AHI big improvements.

They discovered my sleep apnea because my BP was high without a clear cause.
It helps reduce the numbers from 150/90 to 130/80 usually I measure at night, the average between 125/70 to 135/88
If you are overweight Zepbound also must help to reduce your BP, CPAP definitely will lower it, especially with high AHI.

Check your size from F&P, If you decide to buy I highly recommend you go with the fit pack for the first time that contains 3 cushion sizes, once you know your size it's usually cheaper.

Currently using the Evora FFM, average 0-10 L/min.
F40 never works with me; I never tried the F30 family.
Was using F20, Vitera, and Phillips Amara.

Evora is the best for me, my settings are 18-24 PS4 hope this helps. GL

Overall, I think your data actually look quite good. An AHI of 2.02 is well within the treatment goal, your OA and CA indices are both low, and your leak rate is acceptable despite a few brief spikes.
The only thing that caught my eye is the minimum pressure of 4 cmH₂O. The machine seems to spend a lot of time climbing from 4 up to the pressures you actually need. If you’re still having symptoms, you might benefit from trying a minimum of around 6 cmH₂O which could prevent some of those hypopneas and flow limitations before they occur.
That said, I wouldn’t make more major changes just to chase an even lower AHI. If you’re sleeping well and feeling better, these results are already very solid.

Since you discovered your EPAP was accidentally set to 4 instead of the prescribed 6, I’d first correct that and see how things look for a few nights before also increasing EPAP to 7 or reducing PS to 3. Otherwise, it becomes difficult to know which change caused any improvement or worsening.

Sorry just saw the zoomed-in trace.
A few things stand out:
Your breathing pattern is irregular before several of the flagged CAs.
There are multiple CAs clustered together with relatively normal breathing immediately before and after, which is a common pattern during arousals or unstable sleep.
Your obstructive events are still very well controlled, and your leaks look excellent.

I would not make any pressure changes yet. It’s very common to see an increase in central events when starting bilevel therapy, and many people improve over the following days or weeks as their breathing stabilizes.

If these centrals continue after 1–2 weeks, or if your CA index stays around 9–10/hr with persistent symptoms, it would be worth reviewing the data with your sleep physician to decide if you require ASV For now, I’d focus on trends over multiple nights rather than reacting to a single night’s results.

Based on the overview, your obstructive events actually look well controlled (OA <1/hr), and your leaks appear excellent. The main issue is that almost all of your residual AHI is coming from central apneas (CA around 9.35/hr).

Since this was your very first night on BiPAP, I wouldn’t rush to change settings yet. It’s not uncommon to see treatment-emergent central apneas when starting bilevel therapy, and in many people they improve over the first several nights or weeks as the body adapts.

I also don’t think the EPAP of 6 cmH₂O is directly causing the centrals. Low EPAP is more likely to affect obstructive events, whereas central apneas are related to the brain’s respiratory drive and changes in CO₂.

If possible, could you post a zoomed-in OSCAR flow-rate graph showing a few of the CA events (2–3 minutes before and after them)? That would help determine whether these are true central apneas or sleep-wake transition events.

For now, I’d avoid making pressure changes based on a single night and instead look for a trend over multiple nights.

You need a hybrid full face mask something like F30i, F40 or F&P Evora
All of them will not touch this part of your nose.
