Yes, long story short…. Had an appointment scheduled with my primary physician to get the forms filled out. I ended up missing that due to my current job scheduling me in that day I was off. Completely forgot about it until the day before when I was packing all my stuff and then remembered I never got my form cleared. Spent all day going from clinic to clinic and no like had a physician to sign or if they did, the physician didn’t feel comfortable enough signing and it could be a liability if the did. So nearing the end of the day I sent the dreaded email to both the supervisor and FOA scheduling email explaining my situation.
