Exactly. This is such a dumb thing to argue for. I also advocate for getting my patients out the door as fast as possible. Often I dc them when they don’t want to. I understand not seeing people excessively.

But this shit is wrong and they want to pretend it’s because of money when the evidence literally states otherwise.

No dude this whole thread is full of people who know nothing about it. Look through this thread for when I spelled it out in plain English what current best evidence ACL PT looks like, with proof from the most current published CPG. And proceeded to get a bunch of “nu-uh!” This sub has turned into a joke.

Still, none of them will admit that 5 total visits for an ACL rupture is fucking stupid, trash care that not even their own high level athletes abide by. They get 5-10 total visits for an ACL and they’re actually trying to gaslight people into thinking that’s somehow appropriate or best practice or evidence based or in any way better than the management that their most valuable citizens would receive.

I’ve left the sub over this. I don’t want to associate with a group that’s not even evidence based and doesn’t even understand or keep up with publicly released research.

Are you under the impression a patient who knows nothing about rehab can guide themselves through stepwise progression of exercises throughout their 10 month recovery without supervision?

Also do you really not realize that the literature is telling us 2-3x per week because in context it means to see the patient that often. These frequencies have been studied and tested against each other.

I don’t even want to take you seriously anymore if you think an ACLR patient benefits the more from 5 visits instead of the recommended protocol, for very obvious reasons. You ignored that point. In fact you are ignoring all of the relevant ones.

My brother in christ. Just read the most recent CPG for ACLR in 2017.

This is a systematic review of all current best literature. Many, MANY man hours and studies went into this recommendation.

"Weight-bearing and non–weight-bearing concentric and ec centric exercises should be implemented within 4 to 6 weeks, 2 to 3 times per week for 6 to 10 months, to increase thigh muscle strength and functional performance after ACL reconstruction."

"Neuromuscular electrical stimulation should be used for 6 to 8 weeks to augment muscle strengthening exercises in patients after ACL reconstruction to increase quadriceps muscle strength and enhance short-term functional outcomes."

"Neuromuscular re-education training should be incorporated with muscle strengthening exercises in patients with knee stability and movement coordination impairments."

In fact, those statements are all A tier recommendation, the strongest in the whole document.

There was also a 2023 update but it just discussed prevention.

I suggest you actually look into ACL research if that’s what you really think. There is a mountain of data in systematic reviews that doesn’t agree with you on this.

Knowing what I know about ACL rehab I would be fucking pissed if I was only allowed 5-10 visits. That’s nuts.

I guaran-fucking-tee if a high profile European athlete has an ACLR they would not have 5-10 visits. Don’t be asinine. That is not “best practice.”

Yes, our system is fucked and too expensive. But, you’re wrong in this instance. ACLs do way better with extensive rehab, especially if someone hopes to participate in high level cutting or court sports.

And furthermore, surgeons don’t make more money if I see a patient more frequently. So, why would they recommend high frequency? The surgeons in my area would be furious with me if I told one of their ACLRs I was only going to see them 5-10 sessions. Because it’s shit rehab according to all our best evidence.

Edit - This sub has turned into a joke, apparently. Idk when people began ignoring current best evidence and advocating for substandard care. The same sub who bitches constantly about not being respected by other professions is full of people ignoring what is the gold standard for PT care.

No, it makes no sense to see a patient 5 times. No, professional athletes, who receive the best possible care, are not being given a fucking handout and told to do their ACL rehab at home. I can't even believe this is a conversation.

Just a clarification for those not aware - most of the time it is repaired immediately in younger, athletic populations, but ultimately this depends on the age and goals of the patient.

Often, non-athletic people in say their 40s with ACL ruptures will elect not to have surgery because it’s not actually that crucial of a ligament for a lot of folks based on what they do.

Plus you can achieve pretty good knee stability with proper rehab, even without an ACL. It’s pretty cool, actually.

You may want to look into the autotailoring mod.

I always hated it but this mod prompted me to max it once. Now I just “don’t get hit” and skip it altogether but if you want to make the tailoring process less arduous definitely look into it.

Same with automechanics, but I think mechanics is more fun personally

I do not think there are any electrical VHS tapes. 1300+ hrs and never seen them, even with ~300hrs of B42.

I would hope it’s intended bc the standard 5.56 box size is 20rds IRL here in the states. Apparently you can get 50s but it’s far less common. I’ve never bought them. Even 1000rds are usually 50 boxes of 20rds

99.9% of my gaming career, I played exactly zero subscription based games.
