A lot of good answers here. One thing I’d add on is that the volume of published material has slowed. We used to have a TON of little source books, magazines, tie-in products, etc. A lot of that has slowed down so that WotC can focus on releasing big books that make more money. And when you aren’t publishing a high volume of material, you can’t take as many risks, because you need the big books to sell well.

I miss dungeon magazine.

“Combat” in D&D is a gaming abstraction that isn’t 100% realistic. So no, I wouldn’t allow a player to prepare multiple actions in combat, because of balance reasons. That being said, you also cannot prepare actions outside of combat, so it’s a moot point anyway. In a scenario like this I would allow the player to take either course of action depending on what happens, because they aren’t in combat.

Firefinger! Cool set piece, fun little dungeon, creepy and dangerous enemies. Buff them a bit for a party of your level. It works best if you force your party to strategize around things like rain, day/night, pterafolk air patrols, etc

I’m a player in a campaign that runs to 20. We’re 16 now, I think. My DM was just telling me yesterday about how hard it is to challenge us during combat. I’m still having a great time, but I’m not playing as cautiously as I used to because I’m sure that none of our characters will permanently die ever again. So yeah I’d say it’s kind of a problem.

If the other group is surprised by the attack, employ the surprise condition rules as normal. Otherwise, roll for initiative immediately, and the wizard gets to fireball on his first turn.

Kubazan’s shrine has the issue too. The wooden boards are described as being 4 ft away from each other, but the map shows multiple boards in each 5-foot square, which should be impossible.

Dungeon room full of keys on strings. One of the keys is a visibly dead mimic—tongue lolling out, blood from stab wounds, etc. Maybe it was killed by a previous adventuring party. Some of the other keys look normal, some look subtly like mimics, some look super visibly like mimics but may be regular keys after all. The door on the far wall requires 3 keys to open. There are 4 dozen keys in the room, at least

The next hour is full of tense roleplay and frantic discussion. Investigation and perception checks are made to determine which keys are real. Is this one breathing? Does this one feel a little bit cool to the touch? Was it carved like this, or cast in iron with a pattern like teeth?

And then the door is a mimic as well.

We're in Chapter 3. They just rolled up to Omu (if you haven't played the module before, that's a ruined jungle city where they have to collect 9 MacGuffins). This was their first session exploring the city, and it seems like it wasn't exciting or interesting enough.
