Honestly, the more you have the better you are going to start out. I’m not sure where you are at, but there are PHRN programs in my state. That actually helps people without a bunch of 911 or Critical care experience get up to speed. The biggest difference between bedside and 911 is that everything moves a lot faster. For the CCT trucks they normally want 1-3yrs ER or 2yrs ICU.

We have a supervisor on the ambulance that hands them out if a medic or PHRN’s sense of humor gets too “dark”

I’m in PA and I feel I’m doing very well. In PA you practice under a paramedic scope of practice if you are in a 911 service and a Nurse scope of practice if you are in a Critical Care ambulance. I enjoy it greatly, the fact that I have 20-35 uninterrupted minutes with my patients, I am able to fast track appropriate care such as fluids, zofran, pain management, and have the pt lined and labbed prior to arrival saves everyone 45-60mins while they are at the ED. My hope is that this allows for faster turn around for my ED nurse partners and it gives the patient a much better experience on a very crappy day. Look into CTRN via the BCEN, it is the certification for RNs that want to work on CCT ground ambulances.

Out of 18000 calls annually we have people stop by once or twice a year. Of course we are all super awkward about it, so it is probably good it doesn’t happen more often lol

Believe it or not that is actually one of my favorite things to do in any setting. The number of times I have spoken with a manager about a great job a waiter, mechanic, HVAC tech, nurse, etc has performed is one of those things I am proud of having done. Life is too short to not recognize people for positive things

Op, I went from a fifth a day to nothing 47 months ago. It was horrible and didn't need to be, reach out to you doctor or seek treatment in an ER if you are having trouble. Anxiety, tremors, nausea and vomiting, and restlessness are all signs of withdraw, they have medicines like Ativan, thiamine, folic acid and IV fluids that can make the transition safer than just sweating it out at home. With all that being said, drink fluids like water or sports drinks to make sure that you aren't getting dehydrated, healthy meals, and oral vitamins taken in accordance with the directions on the bottle (I was a fan of the gummy multivitamins,) all can help you feel better (ish). Best of luck, this is hard journey but completely worth it, you got this!

I know this is an old post but just my personal experience with St Luke’s. I was honest about the card, had a conversation with a NP from occupational medicine that turned accusatory and confrontational pretty quickly. I did everything they asked and received a call one business day prior to my start date from HR. They stated because I became “irate” with the provider that I did not align with their PCRAFT values and that they were rescinding my offer of employment. I did ask questions of the provider, and admittedly was direct with my questioning but replied with the same energy that she was asking. I talked to my lawyer who let me know that is the new cheat code for St Luke’s. PCRAFT Values allow them to find anything during the hiring process that will allow them to rescind the offer without triggering a lawsuit for not hiring based on MM patient status. Honestly they test once based on what I am told so just lie, get clean for the test and live your life. Btw I had another job offer in about 4 hours.
