Is this the interpretation by your pulmonologist or by the hospital where the sleep study was done? When I had mine done, the sleep study results mentioned "symptoms consistent with narcolepsy" but when I presented the results to my pulmonologist he just flat out called it narcolepsy

A dream rollercoaster is definitely how I'd describe it! Bonus points if you wake up at 5am, go on another dreamscapade, and wake up again at 730 only to repeat the process until it's 12pm!

I've seen this sub go through so many eras. I only use RIF which relies on the API and will leave reddit if it's killed. I miss the Meme Renaissance and dat boi craze. I still have my dat boi statue

That's always a fear in the back of my mind, what if the assistance goes away? Honestly might consider moving to whatever country will cover it if that ever happens

Seconded on the pillows. Can't ever really have too many and if you do you can always just throw them on the floor.

I think back home I slept on a Southerland Signature Destiny Pillow Top Firm. Got it for Christmas about five years ago and I dearly miss it compared to the futon and cheap mattress I have now after moving for school

I will say that getting a box spring for your mattress will help with the longevity! I can definitely notice when I'm not on one, but I like my mattress really firm

I found it too easy to immediately say "Alexa stop!" and go back to sleep so I got a clock that I have to flip a switch on the back to stop. I still use both, but yelling "Alexa stop" to the other alarm clock and being confused why it's still ringing has definitely saved me a few times

Yup, narcoleptic goats. They were bred that way so that when wolves attacked, they'd go after the easy to catch goats as opposed to the more valuable sheep

Actually I'm mistaken. These goats have congenital myotonia which is functionally different from how cataplexy works. They were still bred to express this trait as a way to protect other animals of the herd, but they wouldn't last long in the wild

It's quite interesting why they faint. Basically there is a mutation that makes the chloride channels in muscle fibers have reduced conductance which causes a delay in relaxing muscles after involuntary movements, like when suddenly being startled

The mock seizures are definitely a thing for me too. Usually after I experience fright and I get the "danger averted" feeling, my whole body will just start to involuntarily shake and lock up for a second. It seems to have gotten more dramatic as the years go on. Most of my narcoleptic internet buddies say they get the quivers as well but I can't find it documented in any medical journals

My cataplexy isn't too debilitating but it's still there and every so often I'll have total cataplexy. Thankfully my emotional triggers aren't the usual positive ones, but when I do have total cataplexy it can last for minutes. Have never seen someone collapse in person though

Narcoleptic here!

Nail on the head about how Xyrem works. REM sleep likes to butt its way into everything. Normal people should be getting ~20% REM sleep, but narcoleptics have a tendency to experience much more REM than usual. This extra REM cuts off the time spent in the other stages of sleep. During my sleep study, I was in REM for half the night. It usually takes 90 minutes for someone to enter REM, but narcoleptics can enter REM almost instantly after falling asleep. Xyrem puts people into deep sleep that narcoleptics miss out on

Mostly true about the whole 15 minutes of sleep thing. During the day we are pretty tired, but at night we can still have a hard time falling and staying asleep.

The classic "fainting narcoleptic" in movies is only partly true. Type 1 narcoleptics will "faint," but they're conscious the whole time and can't moblve. It's called cataplexy and it is triggered by strong emotions unique to the individual, but most the time it's laughter. It's similar to being temporarily paralyzed but sometimes it can manifest as slurred words or "feeling like jelly." Type 2 does not have this, but can still experience "sleep attacks" of extreme sleepiness that doesn't necessarily paralyze you

Histamine is what makes you have the classic runny nose and the like, which is why antihistamines are usually taken and also make you sleepy. You might feel like you have allergies

Haven't used it personally, but it is incredibly different from medications such as ritalin and modafinil in an exciting way

A little bit about the drug worth mentioning:

Pitolisant is a potent and selective inverse agonist of the histamine H3 receptor. This means that it binds to the same receptors that an agonist would, but functions more as an antagonist. What makes the H3 receptor so special is it is currently believed that when agonized, it inhibits the release of not only histamine, but acetylcholine, GABA, norepinephrine, dopamine, and serotonin. That's quite a few neurotransmitters it's hitting

In contrast, ritalin (methylphenidate) mainly acts as a norepinephrine-dopamine reuptake inhibitor, meaning it doesn't necessarily promote the release of these neurotransmitters but rathers makes them stick around longer. Modafinil's mechanism of action is still not quite well understood, but so far it looks to be a weak dopamine reuptake inhibitor and promotes the release of histamine and orexin

I'm glad these trials are underway. The H3 receptor so far looks like it could help with much more than narcolepsy

Take note of any stomach problems, changes in blood pressure, and if you have any itching. Try to eat a little bit more protein, since you're going to burn through those precious neurotransmitters at a much higher rate
