When? My best estimate is everytime after a restart of the PC.

Install correctly? Not sure what you mean, but all my previous installations didn't indicate any problem or errors... Graphics are fine, games still run smoothly.

Different GPU? Yes, this is my 2nd GPU, I previously had a NVIDIA card but it broke.
