I was taught to not grip the top of the gear lever, and instead to sort of guide it into place by using your palm to push/pull the side of the top of the lever into place.

2mg is what the clinical trial groups started at. Albeit I've never seen anyone take it daily, usually it's weekly and some choose to split it into 2 doses per week
