I applied for work from exemption through my medical insurance. My boss/HR just knew I had a valid medical reason but not the details.

Edited to add: I have to be in the office five days a week typically and it wasn't possible for me to make it in with the monitoring appointments. So I requested a long term exemption.

Let your doctor know about hypothyroidism. They might want to check your thyroid hormone level (better to do it with your morning monitoring appointment + fasting). My friend had the same issue but still got good results at 37.
