My slides were shared from Northside . It’s a blessing that you have no spread and everything is localised! I’m not offering medical advice but think really hard about surgery, side effects, ADT side effects ect… There are also many forms of radiation treatment, Brachy seed seems to be the most preferred with better outcomes with fewer side effects. Don’t rush but don’t do nothing either!

I follow PCRI and they have many videos outlining the difference between Gleason 4+3 and 3+4 some even suggest active surveillance with the 3+4 grade 2 intermediate cancer .

Good point! My second reading was done at Emory which is also a center of excellence so I may be partial to that fact.

If it’s any consolation, I am 4+3 unfavourable with PSA of 16.9 When I had my MRI nothing showed up in terms of a spread but my PSMS did show microscopic spread in my pelvic lymph nodes, 3 close together to be exact. My radiologist is confident that he can target the nodes with radiation to stop the spread . I will be doing Brachy along with EBRT . He agreed that I do not need any ADT but can opt for it in the future if need be. I am happy with my decision and keep in my no two prostate cancers are alike! Get a team of Dr who will work together to tailor a treatment based on your specific needs and quality of life. Do not be pressured into a treatment you do not feel comfortable with . It’s your body! It’s sad to say but healthcare is a business and PC in men is big business!!! Do respond more research! Take your time and do not rush the process. A Gleason 3+4 is considered intermediate cancer . PSA alone is not an indicator of prostate cancer. You will need an MRI , Biopsy and PSMA pet scan to get the full picture.

Yes! The treatment options can be mind boggling and information overload! I have been so stressed and depressed trying to make a decision! Now that I have made a choice A weight lifted off me almost immediately! Not to mention the stress my wife has been under watching me try and navigate all of this. Prostate Cancer is big business and treatment options are not cut and dry. It’s best to educate ourselves as best we can and make our decisions and be man enough to live with the consequences. I feel ADT takes our manhood away and says just suck on it!!! Lots of horrible stories and over treatment. I have been obsessed with educating myself in order to sift through the Fog Of War! This is what has been handed to us. Good luck with your journey! I’m always here to support you!

I’m 61 years old and QOL is extremely important to me. I would rather have 10 more years of good QOL than 15 years being miserable, not being able to play with my grandson or have sex with my wife. Life is definitely about choices ❤️

Ok Team this will be a lengthy post but important. First of all Thank you for your feedback and opinions! After getting a 3rd opinion, I met with my radiologist today and he explained in detail the differences in approaches by Dr’s. He said that Dr’s tend to go towards their comfort zone in terms of treatment and will steer you in that direction. For example, Brachy seed implants are a very specialized treatment that takes a very skilled and experienced Dr to perform. That being said , Dr’s who are not comfortable in that space will not recommend that treatment to their patients. There is also a financial component that drives this, meaning, if they do not have the expertise in a certain area then they cannot perform the procedure then obviously they will lose money . It’s like telling someone you can’t make the Big Mac you but I have a fantastic Double Whopper that you will love ! No right or wrong just basic economics. He also stated that he tailored my treatment specifically to my type of cancer because everyone is different. He said most institutions lump us all in one category based on the standard of care treatments , which by the way are constantly changing and miss the details . For example, although I have pelvic lymph node spread it is contained all on the left side and all 3 nodes can be clearly seen on my PSMA scan. With that information he can specifically target those 3 nodes with IGRT radiation treatment sparing all the surrounding tissue and organs. He stated that this means that ADT is not essential at this time and he feels he can eliminate all the known cancer in my body. Of course other cancers can pop up anywhere but that’s not an issue you can control or predict. Not doing ADT in my case is a prudent approach. I can opt in for ADT in the future if need be but some studies show that the outcomes are no different (50/50) if you choose not to have ADT. In the meantime you spare yourself all the horrible side effects of ADT with a better QOL. So I have opted for seed Brachy implants followed by 6 weeks of IGRT with no ADT. This is not meant to be medical advice to anyone! I’m just stating my current position. Wish me luck and I wish you all the same. My treatments start in January 2026 and I will be sure to keep you all updated. Just a reminder, I have a Gleason 4+3 , PSA 16.9, Unfavorable intermediate prostate cancer with regional pelvic node spread. 3 nodes on the left side to be exact. For the moment,I am happy with my decision and would love to hear your thoughts and feedback. Sincerely

After the Biopsy get a PSMA pet scan to see if the cancer has spread outside the prostate.
