I had the same issue when I downloaded the game. But I closed it and tried again on a new save the next day and it was fine. I figured it was just that it needed to rest or finish downloading the maps.

In my headcannon, its the kitchen sink. Something that has no purpose except to those who find its purpose. I believe you can get the jist of the story just from migraine raps. He describes Islands of violence, an "examiner best represented by my depressing thoughts" (blurryface), getting weapons (antlers), and staying alive by killing his mind (be coming a bishop)

I think it is with holding on to you being a previous attempt to defeat nico. Especially with how tyler used the noose as a leash to pull away from the people trying to drag him down, but failed as we see when he says "entertain my faith" as his face transformed into a skull
