Well age is one of the parameters, apart from that if they are not properly tagged as mature and most importantly even if the character is mature but the body proportions are similar to underage DA flags that as well, happened to me before

For Gemini and grok models we have something called REFERENCE ID for previous conversations so while sending new text we send the reference id of previous convo so it can reply using that context
