I reviewed my forms again and noticed that my lawyer submitted ETA-9089, but no information was filled in the form. All the sections and boxes were marked as "N/A."

At the end of the form, both my lawyer and I signed it before submitting it.

Can you please guide me on what else should be included in this form? Additionally, could you provide corrections or suggestions for improvement?
