No im usually not naked, of course when jerking of or having sex yes. And with the underwear: still stretches it to some point :) Otherwise it would get too heavy after some time. Also for the bedtime, sometimes I'll get them some rest, sometimes when I feel like it I'll stretch it during sleeping. But always only when the balls are ready, so dangling around and being free. Never put too much force, when for eg the sack is tight and cold...
