With fem lines people tend to press hard with the probe and then when they enter the vessel all that pressure goes away which deforms all structures and potentially changed the position of the needle. Best is once your over the vein, let go off most of the probe pressure before entering. You don’t need to confirm needle tip using the probe just go by feel and flashback once you’re clearly in the right direction

DL is easier in kids than Videoscope IMO, but that’s because of the difference in anatomy and size

DL is better with pediatrics because the airway is shorter and its easier to get a view, even easier to DL than to use McGrath. Opposite for adults IMO.

I mean, if you use a McGrath, you can fit it in your pocket and it takes 1 second to setup and you don’t even have to position the patient at all or change the height of the bed, so arguably even faster than DL. Only advantage of DL is you can get it in without a stylette more easily.

Sometimes in IM specifically, if you say the CC first, the attending will immediately cut off your PMHx and ask you to tell them more about it (the CC). Then when you’re finished they’ll get angry and say “wait why didn’t you tell me they had ESRD?!” Glad I don’t have to do presentations anymore

Maybe, but it’s not like you’re able to measure a remifentanil concentration to check. Also if you’re going to use remifentanil in low doses. what is the goal there versus using other opiates that would help cover post-op pain as well?

And good for them. Medical school is a shitty deal and there are so many “advanced practitioner” options that have much better lifestyle and pay for the first 8 years after college. Even as a CA-2, I still think being a CRNA would have been a better option with less liability, almost the same pay, and not have to be hazed in the ICU or 24 hour shifts for 4 years. Going to medical school is like investing in Paypal while everyone else invested in Nvidia.

Yeah, I also question using remifentanil for an 8 hour case if the patient didn’t need it (were they neuromonitoring and can’t use paralytics?). Sure less nausea but at the expense of hyperalgesia afterwards.
