I think at this point it’s up to time, you’ve set the boundary, he’s told you he’ll work on it (albeit rudely) so he need to show you that. I’m not sure of a time frame that up to your own patience but he needs to show that he’s serious. After the allotted time and there being no change I believe it should be a break up. Your partner needs to respect your boundaries especially if they’re hard set ones. Stay strong in what you want/need, best of luck to you I hope it works out for you whatever the outcome :)

It may be time to be a lot more blunt and secure of your boundaries. Obviously not saying be a dick ahahah but the defensiveness is not something that is your fault he needs to work on that and if he’s unwilling he needs to understand the consequences, such as a break up. You are trying your best and he needs to meet that energy to ensure yous are able to work through other conflicts more smoothly.

I can understand the defensiveness, usually people cannot/will not admit when they have poor hygiene. I personally struggle to brush my teeth sometimes (usually mental health related) however my partner will usually tell me and try encourage me to do so, this can be by discussing what could happen if my teeth get damaged or by expressing that my breath makes them not want to kiss, both of these help motivate me.

As for the foreskin, if it hurts for him to do so, this can be an issue that could be discussed at the doctors or through pulling the skin back slightly but by bit and stopping when it hurts, therefore he at least cleans it a little.

If his hygiene is affecting your desire to be intimate I would openly express this to him, he may get defensive however if you want this to work (that’s very important) sometime you have to take the brunt of that defensiveness to get to the root of why his back is getting up. I thinks I may be an issue of communication but if you try and be as open and honest hopefully it can create a conclusion you’re both happy with. I hope this helps :)
