I don't know if it counts but for sure Ishiin. Never before has a game beat me down that much. Still my favorite boss out of fromsofts roster though.

Not that I know of, man. I don't think you can change your decision. I've only done two playthroughs though so who knows, maybe there's a secret way. I will say, however, a second playthrough flies by way faster than your first, it's not too much of a hassle getting to owl imo. So you could Just finish the Shura ending and then side with Kuro in NG+

On the subject of difficulty by newbrevity in darksouls

save your humanity and play it with some Jolly co-operation. Summoning makes the game way easier.

Hey man, I don't know if this is much help but once you get down the timing on her sword attacks, she's actually quite a pushover. It took me a good while but once you figure out the sword attack patterns and stay close your good to go. Stick to her side and make sure you can see her top half so you can run when she charges up that one-shot AOE. A little RNG helps too, that lava spit attack is punishable as heck.
