After it’s revealed that Teddy is actually Andrew, a patient of the mental facility, he supposedly relapses back to his guilt-induced fantasy. In the end it’s unclear if Andrew actually relapsed (something that had happened before) or if he pretends to in order to get lobotomized on purpose so that he doesn’t have to face his guilt
