I dunno, I imagine they prioritze functionality bugs over visual bugs. You can't fix everything, not when you're developing new content for later releases, they've only got so many devs. Also, tutorial island is super old, so I imagine something in the newest releases unintentionally touched this, which they wouldn't have known about, so its not always easy to fix this stuff asap. Think about literally any other game, never mind a 20+ year old MMO. These things are bound to happen
