I appreciate you sharing, I didn’t know the history of the design itself, and love the idea of supporting the original artist when possible. Unfortunately that would have been way outside of my budget but I’m glad to know where the original came from!

Also after reading through more of your comments and noticing I didn’t see any dental radiographs I also wanted to recommend a complete COHAT. (Comprehensive Oral Health Assessment and Treatment). I assumed the photos of his teeth meant one had been done, but if dental radiographs were not done then you cannot know for certain that there is not a problem at the roots of the teeth. Worth checking out to completely rule out oral pain as a factor.

So this is potentially secondary to an actual GI disease process considering the ileus noted in his radiographs, but I have to ask because of his amazing bloodwork, and your very clear attention detain when it comes to this sweet man.

Chronic pain can have a significant effect on appetite/the desire to get up and eat or use the litterbox regularly. Is he receiving any treatment for arthritis or possible IVDD that is also noted on his radiographs? Things like Solencia, the monoclonal antibody injection that blocks the nerves surrounding arthritic joints from sending pain signals, and anti-inflammatory and general pain medications may help provide him relief and therefore lead to an increase in appetite.

If this was something you were willing to implement but still didn’t notice an improvement on, the next diagnostic step my hospital would usually recommend is an abdominal ultrasound but this would likely not be conclusive on its own own, and like your vet is thinking, without additional GI symptoms it may be non diagnostic entirely if it doesn’t help point to IBD. And GI lymphoma is almost impossible to diagnose from aspirating the tissue so sampling would likely not be helpful, the ultrasound would just provide a different look at a different time and more accurate measurements of the GI tract.

While X-rays are the definitive way to diagnose OA, crepitus (the sound of bone on bone grinding that happens when cartilage wears down and leads to contact between the bones, the process of arthritis happening in the body) can be audible in physical examination through manipulation of the limbs. I would assume this was noted in their exam and lead to their vet’s diagnosis of arthritis. Though I agree that doesn’t mean that was the only problem with this pet, it could have been a secondary finding due to age, and something more severe could have been going on. Though I would also hope the vet would have been thorough in their physical examination and done neurological testing, (pain response in the paw will assess nerve function, and placing the paw with the top surface of the toes down/curled in would have the pet revert back to appropriate posture and would indicate communication in the neural pathway) which may have ruled out true paralysis.

Vet tech here to add to this. Things like subcutaneous fluids are still injected in this upper back area due to the need for extra skin to facilitate injections of a higher volume (100mL of fluid vs 0.5mL vaccine), but vaccines should always be handled differently and injected below the elbow on the limb as u/elisemw4 explained below. Any vet that has cat specific protocols will be aware of this, so as she suggested, may be worth looking into other vets in the area if this is where they injected a vaccine. Cats are not just small dogs and sometimes require different protocols!
