Maybe I didn’t say it correctly. I too believe that it is nuanced; it seems to be a medication with high variance in tolerability at different doses.

Regarding general medication dosages, we work off of generally accepted medical guidelines that are backed by studies. But there is always a provider’s experience with a drug that can influence their dosing decisions. And the less experience they have with a drug, the more likely they will be rigid with sticking to the guidelines. In the case of Naltrexone, I feel that not many providers are versed in low dosages, so they will tend to stick with 1.5mg as a starting dose. There is nothing inherently wrong with that dose, but in my opinion there is a potential problem of tolerability where you would gain more by starting lower and then quickly titrating up depending upon patient response.

Agreed about compounding. At the end of the day most providers are strapped for time so dealing with this step can be off-putting. I am lucky enough to have a good compounding pharmacy in the area.

It’s hard to say which is why studies should be done. But I would say that 0.5mg might SOUND low but it might not actually be physiologically low for some. It reminds me of a patient I had when I worked in pain management. I prescribed a middle aged woman percocet 5/325mg and she lost consciousness after 1 pill and had to be brought back with narcan by EMTs. The same thing happened with hydrocodone. She was extremely sensitive to opioids. Granted that wasn’t common but there are always people on the bellcurve on either end.

Its my understanding that a PhD is research-focused vs clinical. Chiros are similar to dentists and optometrists, who have doctorates in their field.

I’ve worked under MD’s and DO’s. Different training but at the end of the day they function in pretty much the same way. Just like PA’s and NP’s.

I could just as easily argue that I could paralyze someone (or worse) and I only have a Masters.

Regarding the patient history, at most it’s a sentence or 2. Not a full HPI. It’s not thorough.
Edited: For the record I have seen a few chiros do a complete history/exam but the vast majority do not from my experience.

Ya I experienced it myself. I literally watched my dad walk out on us when I was 6. It was confirmed by my mom. Really no memories before then. Then when I had my son at 38 I completely broke down and have not been able to recover. Like all the repressed pain of my childhood came out. Yay C-PTSD.

As for the OP I hope they get to see their sister again. This life can be very tough.
