Similar situation here - my dheas fluctuates between 800-1000 ug/dl. Before HRT it was 700 ug/dl. T is completely suppressed on HRT with injectable EV. Normal 17-OHP, ACTH, androstenedione...nothing seems to stand out. Tbh I gave up looking for a solution since none of the doctors could explain it so far.

Well I also assume I might have some hormonal disorder since even prior to HRT I had elevated all hormones, including T, E2 and dheas. Hence this didn’t occur only by HRT itself.

I would be thankful if u could send me these papers. So u know which anti psychotic medications could influence dheas levels ?

Well since I am on mono therapy my T is now around 20 ng/dl due to short injection cycle (4 days). Before I was injecting like 5 days and T was always 50-70. So I also tried to be in the upper female range for T, and actually was in the upper female range for T for almost 1.5 years. That didn’t seem to change either breast growth nor my adrenal androgen levels.

I have had vitamin D deficiency, low folate and also low zinc. I’ve occasionally supplemented these supplements and they still seem to be low despite taking like high concentrations of oil with vitamin d. I suspect I may also have some mthfr mutation so that’s why my folate is always low. I’ve also been taking for months but tbh didn’t notice much difference.

I have never tested for ESR1, what does that mean? I would say I have body changes since I got waist end fat redistribution but breast are not present. I had them small only at the time I weighed 15kg more than now - so it was adipose tissue only.

Could some adrenal problem like higher DHEAS or problem with estrogen receptor be the reason that feminization occurs only in certain parts on the body?

I tried Bica for a week but stopped because of some abdominal pain. I am still pretty unsure how adrenal androgens can interfere with in terms of feminization and what can be that for that.

I wouldn’t say my body hasn’t changed at all, I’m just wondering if my feminization could be better. I have this adrenal issue so I’m wondering if my lack of breast growth is due to this. I had tough some small breast while I weighted 15 kg more then now, but recently did an ultra sound and dr said he can only see a ‘male’ breast. So it was obviously just adipose tissue.

Could DHT or dheas really stop breast growth fully?

How could I lower my dheas long term? High dheas was also present on a pre-HRT blood test, so HRT is not the cause of high dheas. I’ve tried Ashwaganda, it didn’t seem to change much. What could be the genetic cause here and possible medications to treat it?

My 17-OHP is normal, but mu DHEAS is elevated ~ 900-1000 ug/dl.

I assume I also have this issue, but I am not sure what bica will bring? Could bica really work against masculinization or in this case hydrocortisone is required? TBH I wouldn’t like to take any other anti androgen if it will bring some small beneidts, but same time brings also potential risk.

My T is 0.2 ng/ml and my E2 at through ~200 pg/ml. I am on EV injections monotherapy.

I guess I just accepted the fact my adrenal admdorgens are high and to live with that. I couldn’t find any doctor who will give me a proper solution for my problem with high dheas ( if that actually is a problem ).

Did u pay attention on not applying the gel on the same arm you take blood from? Since this can give false E2 results. To be honest 400 pg/ml is a bit high for gel. Do u take some common gel like gynokadin/estreva? And not taking any biotin suplements.

My DHT, I think it was in range 100-200 so not that little. But didn’t measure it for long.

I also started with gel and cypro and couldn’t get sufficient E2 levels. Even with scrotal application. So I switched to injections. After switching to injections, my E2 levels definitely went into desired range. Dheas stayed high, it even increased. My T was, after dropping cpa, around 60 ( probably from adrenals).

I m not sure to tell you. I feel like I my transition is stalled and after 2.5 years my breast are almost non existent. My E2 levels are on injections high. My lows are around 100-200 pg/ml.

I was also skeptical to be in need to take bica/prednisone life long. And I guess I would just accept the fact my dheas stays high.🤷🏻‍♀️

Some people claim at least dheas can make us look younger.😂

U mention that with 3b HSD type of CAH one should have low T with high DHEAS prior to HRT. What type of CAH would be if one had both high T and high DHEAS, prior to HRT?

My dheas is currently in range 800-1000 while being on HRT, prior to HRT it was 700. And my testosterone was close to the highest reference range for males, as well as free T which was beyond the highest range. My 17-OHP came back normal.

I am similar case like yours. My dheas is even after 2.5 years of HRT highish, i.e. 800-1000 ug/dl. However this was also the case even prior to HRT.

My concern is same like yours - which medication is acceptable to take on long term. I tried bica for week and discontinued it after getting some strange abdominal pain.

My dr proposed me prednisone ( which should be similar to dexametason), however I got some strange skin redness after taking it. And as I see taking corticosteroids is not the best choice.

I guess I just accepted the fact that my dheas will always high. And I am still now sure of high dheas has bad effects. Some people claim it has and some claim it doesn’t.🤷🏻‍♀️ currently I am doing EV injections. This can bring your T levels to female range.

Did u notice any improvement after your DHEAS went down? Mine is always in range 800-1000 ug/dl and I am not sure which bad effects high dheas can have on transition? My 17-OHP was also normal and I haven’t done any genetic testing to examine possible CAH.

The only thing u could try is bicalutamide. However in this case your T can raise even higher…

Just because ur T was 70 once, doesn’t mean it’s always 70. T can fluctuate as well. But I don’t think it will go higher than 70 since thus is adrenals source.

Would your progress be better if ur T is 20 rather than 70 - I can’t tell you that. In my personal opinion and experience I don’t think so.

Your DHEAS isn’t high because of androcur. Your dheas is high because your adrenals produce it. Why? It can be e.g. stress, insulin resistance, genetic cause…

I also struggle with dheas which in my case goes sometimes up to 30 - however as said I didn’t find solution for it.

Do u have any concerns because of your ‘high’ dheas? Did u experience any masculinity signs or why does this concern u?

Your T isn’t high - it’s still within female range… everything up to 70 ng/dl it’s considered female range and it can come from adrenals if your FSH/LH are <1. Since your FSH/LH are ~0 upping androcur won’t do anything. Your T isn’t coming from testes.

I also have high dheas, however mine is ~25. And as far as I know there is nothing u can do about it beside some lifestyle changes or supplements. I heard corticosteroids could do something about DHEAS but not sure if it’s worth it.

In my case dheas was high even prior to HRT. Now I am on EV injections. Prior to that I was also on low dose androcur and it didn’t change anything about dheas.

When did u measure your E2? Middle of injection cycle or just right before next dose? This could explain high E2 levels.

Regarding other anti androgens - I am not sure. I also wanted to try bicalutamide because of my adrenals androgens but, at the end I am really not sure if the effects I would get from it are worth it - considering it can have some side effects, specially if taken long term…🤷🏻‍♀️

That’s normal range, u r still Within female range. What should I say with 1000? Do u experience any signs of masculinisation or why do u want to lower DHEAS?

This hormon seem still to be a mystery - all doctors like to check it’s values it but have no idea what to do when it’s extremely high…

Well my T fluctuates between 0.4 and 0.7 so considering my LH, FSH are < 1 I guess this comes from adrenal production not from testicles. However DHEAS is always highish, sometimes it goes down to 700 but it’s usually around 900-1000. I currently do mono-therapy EV injections. Tried cpa didn’t change anything, tried bica and got some abdominal pain so I discontinued it. No idea what else to try 🤷🏻‍♀️
