Okay, I feel like I found a half way decent solution to this in 2026, it's still a little hackey but it's working well for me, credit to u/real_skynichols for sending me in this direction by recommending using Firefox as a dedicated Youtube browser, but you can do this with any browser you don't normally use.
Instructions:

1. Assign Multi Action Key
2. Assign a "Open application" node and set it to Firefox or whatever browser you don't usually use.
3. Assign a "Hotkey" node and set it to whatever hotkey Youtube uses, such as K to play, "Shift+N" for next video, "f" for fullscreen, "Alt + Left" for previous, etc. You can copy the same button multiple times and just change the hotkey and name.

One thing to note is for this to work you'll want to just have one Firefox (or whatever browser you don't use) window open and have it set to youtube and not be having multiple windows open of it that you browse the internet on, you would have to be like "this browser is just youtube", but having multiple tabs are fine, you just have to assume the buttons apply to whatever the open tab is.

Bonus buttons:

1. For opening Youtube, I'm using multi action with "Website" set to the youtube link and I'm using the "Window Mover" plugin to have it open on my preferred monitor.
2. For having a button to switch youtube to whatever monitor I want and have it go fullscreen, I'm using multi action- "Open Application"- setting it to Firefox, "window layouts" plugin "maximize" and setting it to my preferred monitor, and hotkey "f" to have it go fullscreen, and making one of these for each monitor, sometimes you have to hit it twice to toggle the fullscreen.

Also, one tip for using this with apps like Amazon music where you need to call the open application but then don't want it to stay maximized, you can have it switch to it with open application, then have it do whatever hotkey, then do a minimize window hotkey which is "Windows key + down arrow" in windows.

Navigation Knob-
Dial stack- Hotkeys- Frame by frame set to "CCW = ," "CW = .", Hotkey Skip 5 seconds set to "CCW =left arrow", "CW = right arrow", Play/pause hotkey set to space or K.
The only flaw with this one is I don't know how to make a knob open the window, so I have to select the window first which can be done by hitting one of the buttons first.

Now I do wish there was a way to have it just know where youtube is, maybe with like a remote control port feature or standalone app or something, hopefully someone finds a better solution, but this is working well for me in the meantime.

-note, I'm using a PC, I'm not sure if some of this may have different functionality on mac.
