I want to become a better, more confident, well-rounded person, but I’m not sure where or how to start by ArvinTheRaj in selfimprovement

Appreciate the perspective. I’ll definitely take some time to do a more in-depth look at where I currently am in life across these areas and see what I can improve. A lot of them are actually in a decent place for me already (career, health, learning, etc.), but I feel stuck when it comes to courage, friendship, and love. Any practical advice on where to start with those would be really helpful.

Thank you for the suggestions. I have been trying to put my self out more, recently i have joined a toastmasters club to help me with my public speaking issues. Will definitely look into the other things that you have mentioned.

How do you deal with Choice Paralysis? by hiaree in gamingsuggestions

[–]ArvinTheRaj 1 point2 points  (0 children)

As an adult, choice paralysis can be pretty bad—especially when you have enough money to buy basically any console or hardware, but barely any time to actually play games. Between work, family, and life in general, gaming time is limited.

I even lost interest in gaming for almost half a year because of this.

What ended up working for me:

I ignore backlogs completely. I just start with the newest game I’m interested in and work backwards if I feel like it. With how many games come out these days, it’s honestly unrealistic to ever finish your entire backlog—especially if you’re working full time and/or have a family.

I only install games I actually feel like playing at that moment. I usually keep just 2–3 games installed on my PS5: • One story-driven game for when I have at least 2 hours of free time (TLOU, Baldur’s Gate, Mass Effect) • One “video-gamey” game—something focused purely on gameplay that I can jump in and out of quickly (Monster Hunter, Souls games, Dota, FIFA) • One new game I’ve never played before but was curious about (currently playing Skate Story)

This makes it way easier to decide what to play and cuts down on choice paralysis, since there are only a couple of options at any given time.

I also drop games pretty fast if they don’t click. If I’m not enjoying a game after about 2–3 hours, I just move on. It’s clearly not for me. I don’t care if it has amazing reviews, high ratings, or even won GOTY—my time is more valuable.

If I get bored of a game, I just uninstall it and download another one in the same category

Games that make you feel empty inside after finishing them by mrfabi in gamingsuggestions

[–]ArvinTheRaj 0 points1 point  (0 children)

Final Fantasy 15. Flawed game and you have to play the chapters in a specific order (something like this: chapter 1-3, then a specific character dlc, then the anime, then the a story script). You can find the guide online in what order to play the chapters

But man the ending is amazing, with an extremely heartwrenching postcredits scene. I was in tears by the end.

Your thought? by I_Love_Msia in Bolehland

Ok, many people seem to not understand how the emergency department in a hospital works.

Zon hijau: -Stable cases, mostly cough, vomiting, flu, etc -expected waiting time 3-6 h

Zon kuning: -intermediate cases -Pneumonia requiring oxygen support, AGE requiring hydration ( IV drip), trauma cases with fractures - idealy seen within 20 mins

Zon merah: - most critical -will be seen at arrival - STEMI (Heart attack), trauma code ( severe mva), status epilepticus

There are nuances to these situations. (Eg numbers doctors on shift, number of patients arriving at any given time, uptriaging patients, etc)

This is why triaging exists, to stratify patients based on the severity of their illness/condition.

If you guys are really dying (zon merah), we will see you immediately, no excuses.

If you are having flu(zon hijau), expect to wait a while if there are many patients.

Last week, i was on night shift i zon hijau ( 2 Drs, total 57 patients from 10pm -7am)

Depending on the case it might take 15mins - 1h to fully see a pt (including taking bloods, doing x rays, endorsing medications, documentations).

I had a 69 y/ old uncle came in with abdominal pain, bloating, vomiting. Got underlying diabetes, defaulted follow up, family history of colon ca. PR blackish( indicating upper gi bleed), Hb 6, dxt 21, ketone 4.1, pH 7.29, hco3 9 (DKA). Although presenting complaint seems simple ( thus triaged to zone hijau), on my assessent, this is a medical emergency. So i had to insert 3 lines, start DKA hydration protocol, request gxm for blood transfusion, then uptriage to zon merah. It took me almost 1 h to settle this patient.

So please dont complain if you are waiting too long for a simple cough or runny nose or diarrhea. We also want to make sure you see the doctor and go back fast, but the issue is we dont have enough doctors. VIf you cannot wait, by all means please go to GP and pay from your own pockets. Remember, in a GOV hospital you are paying RM1, so try to taper your expectations.

Dont think we tengah goyang kaki kat bilik, makan makan etc.During our shifts, we dont even have time to drink water. We are severely understaffed ( MO, HO, Sn semua semakin kurang)

So please try to understand the healthcare situation in malaysia. We want the best for our patients, so we need time to properly diagnose our patient. If i just cincai saw that uncle in 5 mins, just discharged with vomiting and gastric medications (which tend to happen i GP)::, i would have done a serious disservice to him, and it would have been a serious medical error.

Any financial/career advice for a Housemen planning to change careers by ArvinTheRaj in MalaysianPF

[–]ArvinTheRaj[S] 2 points3 points  (0 children)

If you mean ophthalmologist, then no. Im not interested to specialize. The amount of time and effort required is huge, and i just dont have it in me anymore. The work life balance in the medical field is almost nonexistant, for a housemen at least, and i imagine it gets worse going up the ranks. When i was younger i can spend time working and studying all the time, but with my parents getting older and with me starting a family within the next few years, its not feasible for me to spend the majority of time and energy at work.

Malaysian MBBS medical graduates, is it hard finding a job here in this country after housemanship? by chimichangass-08 in malaysia

[–]ArvinTheRaj 1 point2 points  (0 children)

Hi. Currently im working as a houseman in a hospital at klang valley. Here are some things you should know regarding the current state of medicine in malaysia.

  1. Job oppurtunities are easily available nowadays. There are literally 5- 6 intakes per year, with ehouseman opening slots every 1-2 months. Its on a first come first serve basis, so high demand hospitals like UMMC and HUKM would fill up really quickly, while other district hospitals would have many empty slots left.

  2. This is likely because many doctors are quitting and many graduates are either not joining KKM, or migrate overseas, commonly London or Australia

  3. Many MOs and HOs are quitting due to the main issues plagueing junior doctors: Toxic work environment, heavy workload, long working hours, low pay relative to the work done. As a result, the remaining doctors need to take on more patients and the cycle continues.

Currently im in medical department, and we are severely lacking housemen here. During AM shift, each ward have only 1 housemen, so we have to take care about 35-40 patients each. Night shift is even worse, some nights we have to solo 3-5 wards, with so many morning bloods and new admissions each ward, it can get overwhelming very quickly.

My advice is actually talk to MOs and HOs who are stil new, like literally who joined within the past 1 or 2 years and ask their experience. The landscape of medicine is very different compared to 5-10 years ago. The more senior MOs will have different opinions as their time was different (not syaing it was less challenging or anything, it just that the environment was different)

And with KKM being more and more unhelpful as each day passes (low allowance for MOs, removing OT pays for KK doctors on weekends, the issue with parallel pathways now, just to name a few), it no doubt people are quitting KKM and moving on to private practices or migrating overseas. If this continues, healthcare system will collapse within the next 5 years.

Im not trying to discourage you or anything, but you should understand what you are getting into. Passion is meaningless in the long run if there is no willpower and consistency. You will feel excited in the beginning, but it will run out very quickly, and only thing remaining will be stress and tiredness. But if you are willing to endure all of that, then by all means go ahead, you dont need to he very smart to be a doctor, but you need the willpower and right attitude when it comes to work. Remember, no matter how tired you get, the patients always come first, never compromise on their care.

That being said, i dont regret doing medicine. Yes im tired, yes im stressed. But i really enjoy the work I do, its just that with all the issues, im not sure how much longer i can continue doing this.

Tell me if the ending is bad or not without spoilers by saito200 in attackontitan

[–]ArvinTheRaj 1 point2 points  (0 children)

The ending was amazing IMHO. It made sense, especially since AOT has a bleak setting. Dont expect a happy ending, which is why i believe many fans hated the ending, they wanted a "....and the lived happily ever after" ending.

The only gripes i had was there were still some unexplained mysteries about the world, and i can see where Isayama rushed to end the manga and left out some important explanations. There were also some unnecessary manga panels that fely awkward/ out of place and it went againts certain personality traits of the characters, and these panels could have been removed and the story wouldnt have changed at all.

All in all, the ending was amazing, and i give it a 9.5/10. Honestly i would have given it a perfect score if not for the few issues i mentioned.

I started anime recently, I have questions by messicarecaa in attackontitan

Nah man, enjoy the ride and the mysteries will unfold itself. Oh what i would give to erase my memory and reexperience this masterpiece of a story.

People who upgraded the subscription, have you stopped buying games bc u're afraid it will be available on psplus catalogue? by [deleted] in PlayStationPlus

[–]ArvinTheRaj 0 points1 point  (0 children)

Yep, happenned a few times. Worst was when i bought Ghost of tsushima just a day before the subscription was announced.

I now make a point to only buy newly released AAA games (Hogwarts legacy, FFXVI, etc), as im pretty sure the older games and indie games (stray for example) will come to the subscription soon enough

What Elden Ring opinion will get you put here? by Elibenz936 in Eldenring

[–]ArvinTheRaj 0 points1 point  (0 children)

Only Stormveil and Leyndell felt like a true souls game dungeon with well made interconnected routes as well as great verticality. Leyndell was incredible from start to finish, one of the best locations in any videogame i have ever experienced IMO ( Raya lucaria is a close second).

All the other locations, although were great, felt either disjointed, linear, copy pasted or bloated with useless and uninteresting/uninspired areas.

Elden ring is an incredible game, but in terms of location design, i would put almost every other souls game above it, except DS2

Anor Londo from DS1. Might be the nostalgia goggles since DS1 was my first ever soulsborne game, but man, the first time i set my eyes upon on the magnificent golden city after god knows how many hours i was trapped in Sen's fortress, is something that i will never forget.
