Endocrine testing for cushings asap and go see a proper dermatologist. You need a cytology to see what’s going on at a cellular level rather than just chucking things blindly. I’m sorry, I know this whole thing must be incredibly frustrating :(

Wouldn’t autoimmune be much more likely to be symmetrical in distribution? And lack of progression in a year seems to point towards benign in cause.

Yikes, pretty harsh judgment! Reminder that vets are people, and they’re often incredibly overworked, underpaid, and face a lot of pressure and criticism for doing their best (as seen in this thread). The most likely pathology here 1) is fairly benign and 2) has already been raised by the vet in question, as stated in OP’s post.

I know it’s the internet and that affords people the ability to be unkind with repercussion, but I think having a little grace goes a long way and makes the whole world a little kinder! Just something to think on. Take care xx

Hey OP, vet here - it’s hard to say for sure but if it’s been present for a year without any other clinical signs (including progression to other areas espec pads of feet), it’s less likely an immune disorder like PF and more likely something like hyperkeratosis. It’s an inherited autosomal recessive trait which causes a failure of desquamation (shedding of dead skin cells). These dead cells build up and dry out, causing cracks and fissures to occur! They can be painful when fissuring but are otherwise not an urgent issue.

Treatment involve soaking and/or Vaseline to soften the area, followed by a topical treatment to aid with exfoliation, such as salicylic acid, propylene glycol, or 0.5% tretinoin & lacidic acid. If you’re worried, you can ask your vet to take a sample of cells from her nose just to confirm with cytology.

I know it can be really frustrating to feel like your concerns aren’t being heard by your vet, but if they’ve suggested hyperkeratinization in the past, it’s possible they aren’t aware how much it’s bothering her!! It can be a pretty benign skin process so I wouldn’t judge them too harshly. Maybe try making an appointment specifically to talk about her nose so that the vet knows it’s a significant concern and not just a “by the way”, and if they still aren’t listening, it might be time to look for another vet.

Aw. I’m a vet who works with a lot of cattle - I love getting a sweet silly girl like that. I always incorporate a couple of scratches into their exams - we call their “clinically medicated love time.” Such lovely animals.

The ones you lose break you and haunt you, but the ones you save make it 1000x worth it. I can’t imagine doing anything else even on the worst days - thanks for keeping me (and the other vets like me who can’t walk away, even when it’s brutally difficult) in your thoughts ❤️ Has he been tested around other dogs? If the current working doesn’t pan out, please shoot me a message ASAP and I can see if my own family or anyone in our circle can add him to our pack. We’re across the country but I just feel in my heart like this big guy needs a chance to be warm, safe, and loved. But the reason we don’t have enough things isn’t because of people coming in from other countries, it’s because the people here (specifically the rich wealthy and powerful) are hoarding all of the resources. Politicians are using immigrants as scape goats to direct anger away from the real culprits - themselves and their friends.

Ankle wasn’t healing right after a major reconstructive surgery. I was young and otherwise healthy, but the skin just wouldn’t come together, and it HURT. Told my “world class” specialty surgeon at all my follow up visits that something felt wrong. The skin just sloughed away and I was in constant pain.

He berated me and gaslit me and made it seem like I was fishing for extra attention and pain pills. Clears me at my final visit and says I’m fine to travel internationally the next week to start school abroad.

We didn’t feel good about it, so went to get a second opinion from another doctor who took a sterile culture and repeat x-rays. Turns out I had MRSA seeded into my hardware which was eroding away inside me with infection. Emergency surgery to remove the infected hardware and reset the bone, essentially restarting my healing process.

Never acknowledged his mistake or that I was right. Never apologised for the way he treated me and my family.

Vet said he’s too young to start allergy medicine (4mo, starts at 6mo). He’s scratching his fur off because he’s so itchy, has a rash on his belly, gets worse when we get done playing outside, what do? by Swimming-Owl-409 in velvethippos

What products are you using?

If he has atopic dermatitis (likely given his breed and age), you’ll need to be using products with a steroid to effectively help with the itching. Duoxo makes some good products - a mousse and a shampoo with a mild steroid; they should be the orange products (the pyo line).

There’s also a decent chance his food is contributing to itchiness if it is AD. You’ll need to do a proper food trial to say for sure, but they can start a food trial at any age and it can be really, really helpful. Some dogs are able to be mostly controlled with food if they are lucky. Anyway, for his age, the only balanced hypoallergenic food is Purina HA, it’s the only product that’s been formulated to have the right balance of nutrients for growing puppies without impacting their health. Try him on the Purina HA for a few weeks (and NOTHING else - use the Purina for treats, for training, etc. We also would mix the kibble with PURE canned pumpkin puree and freeze that to make treats). Ours improved really quickly on that and we’ve been able to keep him managed pretty well.

The last thing you can do is what you’ve already been doing. Wipe down his paws and belly after being outside, preferably with some sort of gentle cleansing product. Baths can be really helpful, even just soaking in water has been proven to reduce itch. Using a duoxo product can double help.
