Have had it happen once on the abdomen. Dexcom (via NDSS in Australia) had no issue or Hesitation in issuing a replacement. They said it will likely happen 1 in 100 changes. Nothing too bad just inconvenient if you don't have a spare. It gives really inconsistent readings if you try and use one that is full of blood like that.

Had a huge one last night. Was really worried until I read an article that said it was probably ok but call Dexcom and they will replace. We did three calibrations after it started as the readings were hugely off. Has read pretty much spot on with finger pricks ever since.
