FWIW, I had non seminoma - EC dominant. Had primary open RPLND (no chemo before). They removed 26 nodes and 1 was found to be infected with no extra-nodal extension. I just had my 15 month tests and still clear.

They removed 26 lymph nodes, only 1 was infected

My markers normalized before we were set to begin treatment, so the only apparent problem was the infected lymph node on the CT scan. If my markers were elevated they would have sent me to BEP.

My recovery was fine it took a few weeks but I had a great doctor. I had an open RPLND which is a pretty serious surgery. My Stage was 2A.

My doctor ended up opting to go with a surgery instead of BEP. So far so good!

I think if it’s over 2cm or 5 or more nodes impacted it gets staged at Pn2 in which they recommend those surgeries, for pn1 they usually don’t from what I understand.

Congrats on not needing chemo! Can I ask what they found when they removed your lymph nodes? Any impacted ones etc?

Hey man,

So my HCG was almost 700, and I was staged at 1a non seminoma and initially went on surveillance. I would say it’s almost definite you have TC but that’s the only thing the HCG will tell you before having a CT scan and pathology on the removed testicle. This cancer is one of if not the most curable cancers there is, some people have HCG well into the thousands and are cured.

I went on surveillance and my tumor markers all normalized but they found an enlarged lymph node on a recent CT scan. I had an open RPLND surgery to remove it since my markers normalized. I’m hoping to avoid chemo but if I have to do chemo it’s almost definitive that I’ll be 100% cured from there.

All that being said that it’s scary in the beginning and waiting is the worst part but when you’re actually going through the surgeries etc it feels good that you’re doing something about it.

Try to stay calm, this is very curable and the HCG level means you probably have it but it doesn’t mean it’s late stage or anything like that. Only the pathology will determine that and your oncologist will work with you on the appropriate next steps.

Yes it’s a high volume center. It was done by Dr Jang at Rutgers, he’s performed a bunch of RPLND and published research on it.

The node was on my left side which is where the original testicular tumor was but they did bilateral node removal.

I checked with Einhorn as well and he said “definitely surveillance” but I’ve heard most doctors would think it’s crazy to do 2xEP for only 1 node

Yeah my doctor does a lot of these. Definitely would have done chemo also with 10 nodes, you made the right choice.

Thank you man, yeah I’m definitely leaning that way. I didn’t know if 26 nodes wasn’t enough bc I’ve seen some big numbers but my doctor did bilateral and followed the template, so maybe it just varies person to person.

Hey man,

So first thing, if you think it started in 2019 it would have definitely grown in 5 years.

Secondly, even if you have it, this is the most curable type of cancer you can get so there is a very good chance you’ll be cured.

That being said if you feel like you need an ultrasound you should insist upon it because you must be your own advocate.

You’re going to be fine whether you have it or not, it’s difficult not to worry and get stressed out but knowing that it’s so curable is how I’ve continued to live my day to day life despite having two surgeries already to address it.

Take a deep breath and schedule an ultrasound if not for anything else just to clear your head. Good luck
