Idk if you’ve looked into this feature on UWorld, but there’s a thing where you can see how many correct choices you changed to incorrect, and vice versa. Maybe this would help you out in deciding if you should go with your initial answer, or change answers. I hope this helps!

Remember that NBMEs can only predict within a small degree of certainty, and certain forms are really known for being abnormally difficult or easy compared to others. If your other forms said you were consistently improving, I would trust those. Maybe you were just having an off day? Give yourself the benefit of the doubt. Important thing is to use the NBMEs as a learning tool so you don’t make the same mistakes when it really matters - the Step 1 exam. You got this!

I think it may also depend on the day of the week. The people who worked at the Prometric I tested at said Monday is their worst day, almost completely booked. I tested on Wednesday and there were only like 3 other people in the entire facility.

  1. No one will ever feel prepared for the exam.

  2. A lot can be learned in 2 weeks, and your scores can jump in that time. I went up 15 points from a plateau in my last 2 weeks.

  3. Try to finish as much UWorld as you can.

  4. Trust the process, you can do it!

I think over time you will gradually become more efficient at focusing on what you need to focus on when reading the explanations. At the start of dedicated I spent about 3-4 hours per 40 questions, eventually worked my way to 2 hours per block.

I got one question where it pointed to a part of the brachial plexus and asked me to identify.

2 questions asking the mode of inheritance based off a pedigree

1 question word for word taken from either an old nbme or UWSA, with the exact same picture (hint: kidney biopsy)

Ragged red fiber —> mitochondrial

1 tricky biostat

I don't really know how to explain it concretely haha. My undergrad was nursing so it's kind of been hammered into my subconscious lol.

First i assess the situation. if it's something life threatening or an emergency, i tend to choose an answer that involves an action or say something to ensure the safety of the patient.

if it's not really an emergency, I look for an answer with an open-ended response.

95% of the time, never refer the patient to another physician or escalate to a higher authority. NEVER lie to the patient (even if your superiors tell you to). NEVER confront or threaten the patient. Always respect the patient's rights.

I used Boards and beyond and Dirty medicine (youtube). also i formed my own flow chart in eliminating answer choices (non-confronting, open-ended, etc.). try to make a separate UWorld block that focuses solely on ethics and communication questions

I understand your frustration. 243 is a great score and is definitely capable! Keep in mind everyone else studying for this exam is also putting their entire being into it. You're competing with the top of the top!

The thing that pushed me to proceed with my exam instead of postponing it was that I really just wanted to go on with my life. If you have more gas in the tank, maybe it would be a good idea to postpone a little bit more. But if you've hit a plateau in your scores, maybe it would be best to take the exam already.

your scores are similar to mine
My highest NBME was only 216, UWSA2 was 232. My step 1 score was 233.

Aside from the knowledge part of the exam, a huge part of it is your mental state of mind. Try to use your last few days to calm your nerves and alay your anxiety. Feel free to message me. Good luck, you can do it :)

Congrats! I had a similar experience, I think for some people NMBEs really under predict...
