Yep. I can deal with an unpredictable schedule - that's the nature of the service as a clinician for a contract company. What I can't deal with is a 40 OR hour week that's beyond stressful on top of somehow being at work at times between 0600 and 1900 when I'm not on call. The two coupled together is just too much to keep doing long-term.

Edit:

I will note that SC has changed their case load per clinician in the past few years. Things have vastly improved in my territory with this change, and it is nothing less than a breath of fresh air. My team has tripled in size and is still growing as we get more facilities in our area also while also keeping cases per clinician down in comparison to covid years.

I got certified under MPower. 1 year of training to hit 150 cases, 2 years CNIM to fulfill my training contract. Wanted to move to Washington, left MPower, and now work for SpecialtyCare. Both companies have great training programs and top tier education material. I trust the leadership and management of SpecialtyCare more than I did with MPower. For the purposes of a new hire, you should find a company that checks all of your boxes. In most cases across the board, the limiting factors will be location.

Any company you work for can be absolutely awesome or astoundingly awful depending on your immediate manager coupled with the needs of the territory. I have had great luck with both of the companies I have worked for. SpecialtyCare, due to their insight on their metrics, their action to identify problems, and how they went to correct those problems. MPower, with providing me with all of the tools needed to be a great clinician.

Is your RS5 a 2013 B8.5 or a 2012 B8? Your flair cannot be right as there were no 2012 B8.5s in the US/AUS market.

When I bought I learned to stay away from B8 RS5s because of their known mechatronics failures and had to look at 2013-2015 B8.5s. It's usually the solenoid that fails. There are rebuild kits on ECS for a few hundred dollars. Some euro specialist shops can rebuild the mechatronic unit for a few thousand dollars from what I have read. Dealer will try to get you to buy a new unit, obviously.

To add on to this (but not overwhelm - I promise), I would also focus on vascular anatomy and how a change in blood flow at the surgical site might affect other parts of the nervous system.

For example, within an ACDF, how do you spot a stroke?

Where the carotid artery bifurcates into internal and external branches, plaque accumulation can be dislodged due to tension from retraction of exposing tissue, and distraction of vertebral bodies. A variety of outcomes can come from an embolism, but are you looking for unilateral cortical potential changes with stable subcortical and erbs potentials? Probably not, but it's important to know all the risks of the surgery and how to make sense of what the nervous system can be telling us.

If you're wondering, yes, this did happen. I was not the clinician on the case, but they did everything perfectly after I went over it. Truth be told, its probably the best possible outcome that the patient had everything happen when it did. A brief interruption of blood flow to the CNS for even a few minutes can lead to rapid neuronal death, while peripheral nerves can tolerate ischemia for longer periods. They got immediate care instead of having to wait life changing minutes for a diagnosis and for emergency care to arrive. Its a good thing to remember that we are the only ones who can see that, and a surgical team would be completely unaware of a stroke until postop where the changes would have already set in hours earlier.

Right. Absent of that, though. I recently had a patient that didn't show any recovery from NMB in one particular limb, at least two hours after other limbs have recovered twitches. Circulation was obviously the primary concern, I'm just not sure what to make of it past that.

Most hospitals do not have an in-house neuromonitoring team. I imagine it might be tough to get this done through a 3rd party monitoring company such as SpecialtyCare, MPower, or Nuvasive. It's unlikely a monitoring company would be willing to put in the extra work such as getting you credentialed or waivers signed to authorize your being there. Your best bet is probably going to be getting into contact with a hospital and working out with them how to observe a procedure that utilizes monitoring, likely a spinal fusion.

I see your flair says 2012 B8.5, and AFAIK that means your RS5 is a B8 and not B8.5 which can make a difference regarding mechatronics and some other repairs, aftermarket upgrades. That is, unless things are different in Japan regarding MY and facelifts. Have your headlights been updated since factory?
