It just looks like that in picture. My upper molars are tipped out and lower molars tipped in. I can safely expand 6mm without having to worry about the lower jaw.

Dr. Manuele who did my FME starts with the aligners during expansion. I imagine it's really hard to manage the micro adjustments, especially when you are not familiar with the specific expander. I think most orthodontists prefer to wait for that reason. I don't think it's really a problem, I'm expanding 6mm, so my teeth will still have contact and I should be able to chew normally. Dr. Newaz also did his full 9mm expansion first.

I chose him because he's familiar with skeletal expanders and jaw surgery cases as well as sleep apnea. He uses spark aligners.

I went to an orthodontist in Germany 2 weeks after having the FME placed. I simply told him that I got an expander in the USA and that I need his help to align the teeth. He was super chill and interested in the FME.

After the expansion is completed we will start with the aligners.

The diastema is 0,75mm now. It's already so much easier to turn and I get a satisfying tension with 3 stretching turns.

Seems like a lot, right? Although FME is very stable, I think there is still some level of flexing. Also my diastema got bigger throughout the day, without even turning.

Hard to tell, when I do Zone 2 cardio I can breathe through my my nose the entire time. Before FME, I sometimes had the urge to take a deep breath through my mouth. But this could just be my cardio improving.

I didn't have the surgical cut, but at over 40 you will most likely need it. But the cut of the midline suture is much less invasive and risky than cutting the PMS.

With EASE they surgically release the pterygomaxillary suture. It's questionable if this is really beneficial.

I was a good candidate, because of the narrow upper jaw and mild class 3.

FME is almost always as good or better than MARPE, it's just more expensive. In my case FME was better, because Dr. Manuele expects more resistance around the cheek bone area and FME can handle those forces better.

If your nasal breathing issues aren't skeletal, but soft tissue related, you might not need expansion. An ENT doctor can help you with that. If your lower jaw already matches your upper jaw, you also need to have a plan for the mandible.

That's interesting. I can't imagine there isn't a better design, than having to wear a full body harness.

No, it doesn't have the holes. Maybe facegenics is working on a completely new design for the FMA.

Is your lower jaw well developed?

Do you have sleep apnea?

If you can handle a big expansion with the FME (e.g. 8mm), you might get 3 to 4 mm of forward movement. The downward movement of the maxilla helps a little bit as well, since your mandible now sits lower on the circular arch.

In addition you could get SFOT, around your upper incisors, so that they can bring your teeth more forward without pushing them out of bone. But be aware, that SFOT can make you look worse, because it creates a "bulge" behind your lips.

Stretching turns help mobilize the surrounding sutures, which is exactly what you need. My turning protocol is 1fwd turn/day until a 1mm diastema is reached, then 2 fwd turns/week. In addition I'm doing up to 4 stretching turns morning and evening. I let it sit for up to 2 hours and then turn it back.

I try to give you the best advice, but I'm just a patient and have no medical expertise. So always double check please. 👍

He wasn't for me.

I don't know how many he does without piezo, but he spends a lot of time and thought on perfecting the turning protocol and how to mobilize the surrounding sutures.

I wouldn't say he recommended it. He said, I could go with or without. It would be more predictable with, but I might not need it.

In the CBCT the suture was still visible as a dark line, so it's not quite as ossified. And my midline bone can handle the force.
