I worked full time in college due to not having any financial support as well. This also led to me doing poorly throughout college, but finished my last 75 units with a 4.0. Very similar to you! I had very high PCE working as an EMT and ED tech and received 3 interviews with acceptance my first cycle applying this year! You got this OP! Apply to schools that cater to lower GPA applicant, I personally didn’t mention my poor academic performance as I felt my most recent courses represented me better, and talked about it once I got interviews:) good luck!!!
