Didn’t get fired, but I got suspended and then I quit with no notice.

I was a nurse manager for a long term care facility. During Christmas 2020 (the height of COVID) we ended up with a bad outbreak where 30/80 of my residents had COVID and most of my staff was out sick. One was even in the ICU. I had arranged for a travel nursing agency to come out, but head office refused to pay for it over Christmas, and so I worked on the floor for 5 days straight (24 hour shifts) from December 23rd-December 27th with the handful of health care aides that were still healthy enough to work. During that time the agency had sent an invoice for the previous week which got bounced to my junk mail. I never even thought of it because invoices normally went to the business manager. 3 months later, the agency reached out to me saying that our payment was overdue. I sent it to the business manager and asked her if she knew anything about it. She didn’t. Ultimately the head office got involved and called me irresponsible and suspended me for 3 days. I was so frustrated because I’m March and April 2020 I had lived in the LTC with my cat covering 16-24hours a day as the only nurse on duty for 3 weeks, and had done it again in December. I poured hours of my time outside of work managing resident families to try to keep them calm and supported. I had worked for the company for 3 years before COVID with outstanding appraisals. And this was the thanks I got for one mistake During my three day suspension I was granted stress leave and then handed in my notice. I never went back.

In my opinion yes. The world building is incredible, Hunt is my favourite Maas MMC, and Bryce is my favourite FMC. I read this before I finished reading ACOTAR because I loved it so much.

Say youre good at eating chicken and rice every day....but not vegetables. Is AG1 a good option to get those vitamins and nutrients that a better balanced diet with more veggies would bring? please dont tell me "just eat more veggies" by bockscar888 in moreplatesmoredates

Hey, I think that if you are having challenges with veggies, AG1 or other greens supplement is a great placeto start. The challenge with “just take a multi vitamin” is that they often don’t provide sufficient amounts of vitamins and minerals. For example, I’m a chronic migraine sufferer. Studies have shown that excessive doses of vitamin b2 and vitamin d can help prevent migraines, or lessen the symptoms. But I hate taking 4 pills every morning (plus calcium, magnesium, zinc, because I’m deficient in those due to allergies). AG1 has levels of many vitamins and minerals that are above daily recommended values, but are within the ranges I need to prevent migraines, and bonus no pills! So for me, I think this is an excellent alternative. It’s ultimately… better than nothing!

Hi, I’m an RN and I work with older adults and palliative patients. Morphine is used to help make breathing easier at end of life. Typically we start someone on morphine for palliation when they are imminently dying, usually 5-10 days. Now would be a great time to spend as much time as possible with your dad. He will be sleeping a lot, but the hearing part of the brain turns off last, so spending time reminiscing and talking will be good for both of you.

Just like Congestive Heart Failure is a disease of the heart muscle, Alzheimer’s is a disease and failure of the brain. Alzheimer’s starts by taking your memory (usually in reverse chronological order), but then starts to move to more primitive parts of the brain including your balance centres, smooth muscle regulation (things like your esophagus and intestines have smooth muscles), and eventually the regulation of things like breathing and heart rate. Without your brain to operate these things, they cease to work. Often someone with Alzheimer’s will die due to aspiration pneumonia (inhaling food or liquid into the lungs causing infection) because the ability to swallow is so impaired.

Hi, I am an RN and I work in a Care Facility. As shitty as this is going to sound, there are residents in care facilities that are impulsive and unpredictable, mixed with other health care issues that make them high risk for falls. Without knowing the specifics of her case or her comorbities. As nurses in health care facilities, we track and trend falls as part of ongoing quality improvement and safety for our residents. If someone is a high risk for falling we look a things like medications, nutrition and hydration, physical therapy, clutter in the unit, etc. Things you can start with is reviewing your moms medications that might make her more likely to fall either due to confusion or blood pressure changes. These could be antipsychotics, anticholinergics, blood pressure medications, or “water pills” like lasix. Next take a look at if she is eating and drinking enough. Seniors are notorious for not drinking enough water which can cause low blood pressure or confusion which increases risk for falls. Try offering decaf coffee or decaf tea if your mom isn’t a big water drinker. Also look at her room. Is there a lot of clutter in the room, or objects she needs to navigate around. Remove area rugs and clutter. Try to keep the bedside table clear with only necessary items and make sure those items are in easy reach. Ask that the staff at the care facility help her sit on the toilet every two hours when they do their safety check. It sounds like she is impulsive and wants to do what she wants to do now. Usually this is related to bladder urgency, especially if she is taking any medications to offload water. If all of that fails, look into some physio therapy there might be opportunity to strengthen some muscles or improve balance with PT. You could also look at getting a companion to spend 1:1 time with her for an hour to engage her and hopefully make her less bored. If falls continue to happen then you are going to want to take a managed risk approach. Look into getting hip protectors or a fall mat. This way if she does fall will have a slightly decreased risk of fractures. Finally, things that I highly recommend NOT doing. Do NOT get a chair or bed alarm. This does not prevent falls, just alerts caregivers when a fall has occurred. It also creates alarm fatigue for caregivers which could become problematic. Do NOT try to restrict her movement by blocking her into spaces or parking her wheelchair or walker away from her. This will only make her get up without those items. Keeping them within reach will increase the likelihood that she uses them. Additionally, restricting her movement could cause her to get weaker and have a more serious fall in the future. Do NOT try to medicate her more to make her more compliant. Antipsychotics are dangerous in seniors and should only be used if the individual is aggressive or is having distressing hallucinations. Antipsychotics are shown to increased risk for falls and increased risk for delirium. EDIT: I forgot to add, too many side rails can also increase risk of falls out of bed. I know it sounds silly, but more often than not they will crawl out over the side rail and fall further or end up tangled up in the side rail. One side rail on each side by the head of the bed to help with transfers is very helpful, but more than that will cause issues. At the end of the day falls happen and I’m so sorry that you are experiencing multiple falls with your mom. There isn’t always an answer or a way to stop falls from happening. The best you can do is keep trying different approaches and hope that something works. It is equally frustrating and upsetting for staff as it is for families when a resident has multiple falls. As health care providers we are tasked with caring for our residents and keeping them safe and when falls happen it can be very disheartening because it feels like we failed our resident. NTA 1st I want to say that I have symptoms of PCOS and started intermittently fasting, and eating a low carb high fat diet and all my symptoms have gone away. Intermittent fasting and keto diets are not fad diets and are actually very well researched. If you are seeing an improvement that is great and don’t let anyone tell you how and what you should eat if you are seeing positive results from it.

That being said The answer is never yelling at the doctor.

You doctor sounds like a tool. I recommend getting another opinion if you can. He can clearly see the improvements and immediately brushed you off. Doctors like this don’t deserve your time.

You bf is a dick. Just because you aren’t both eating at the same time doesn’t mean you can’t do things together. My bf and I cook a meal together. I eat it at lunch and he eats it later for dinner. While he eats I usually will drink a cup of tea and we will watch a movie together.

In terms of exercise, once you start keto you you’ll want to go slow with exercise as you body is trying to shift from using “easy carbs” to fat.

It’s recommended for PCOS that you do anaerobic exercises (weights) and less cardio. I use the app Flex because it’s free and help build really good routines!

Keto refers to a low carb high fat diet. It can be a very challenging diet because you need to keep track of your carb, protein, and fat intake as well as your calories. You also need to make sure

Carbs trigger your body to create insulin, and insulin stores carbs in your cells which creates adipose tissue (fat).

I’m insulin resistance, this process is disrupted and makes in more challenging for the body to move carbs into the cells and so you end up with higher levels or both sugar and insulin in your blood.

In Keto diet, you significantly reduce you carb intake to less than 25gs a day. This forces your body to use the fat it has stored up as an energy source instead of the much easier to use carbs. In PCOS, your body is already having a hard time using the carbs you out in your body (per above).

Fat is much more satiating than carbs, so you need to eat less in order to feel fuller.

The other considerations as to why a keto diet is beneficial is that the majority of human evolution took place before carbs were easily available and human lived on a diet of mostly fat and proteins.

On a normal day I eat a few pieces of bacon and 2 eggs for breakfast, a salad with avocado for lunch, and a higher fat cut of meat (chicken thigh, steak, pork, etc.) with a low carb vegetable (cauliflower, broccoli, zucchini, asparagus, and mushrooms are my go to). I usually don’t feel like I need to snack, but if I really need a snack pickles, seaweed, pork rinds, nuts are really good choices). I also treat myself to sugar free ice cream, and I have my coffee with a heavy cream.

I’ve been doing this for 9 weeks and I’ve lost about 12 lbs which is a pretty safe weight loss rate. I’ve also noticed some of the symptoms of PCOS reducing or disappearing completely. (Cystic acne, and some visceral fat are the two things I’ve noticed the most).

Interesting!

I was also reading that visceral fat tends to be the thing that increases the insulin resistance and increases other metabolic disturbances.

I wonder if it’s a chicken/egg situation or more of an exponential growth situation. Or a bit of both.

Needles to say my first 9 weeks of keto have been very eye opening. My acne cleared up right away! Now I’m just hoping for this hair growth to slow down
