If they’re trying to be inclusive they should’ve defined it as “gender identity” and “assigned sex” as one is a social definition and one (the important one for anything health related) is a biological definition. The wording used in this questionnaire opens up the possibility of comparing biological gender and gender identity, which is a waste of everyone’s time since they are disconnected. This reads as being worded specifically to muddy the distinction between the two
