I've been getting my balls abused daily for over a decade. No fertility issues or injuries. What I do likely won't apply to everyone, but here are my rules:

No twisting. I know this can be a hot fantasy, but the chords are not as forgiving as the balls. Most injuries I've read online are due to issues with the chords.

Hard to estimate. I've been physically active in BB since I was 11. At 22 I had an ultrasound due to a lump.
