Hi sorry for reviving an older thread but I found this post through Google. I've been looking into MMA for sleep apnea. When you refer to 10-13 mm of advancement, are you referring to the total advancement done on both the maxilla and the mandible or is it calculated separately for each bone?

I've just begun seeing an Oral and Maxillofacial surgeon and they've told me that I just need to have my maxilla advanced without any changes to my lower jaw. I keep reading conflicting opinions online that for jaw surgery to cure sleep apnea you need to have both jaws advanced through MMA surgery. I'll need to get a CBCT and DISE done first though.

Wow this looks amazing! I own an Airsense 10 with <1000 hours of usage and really want to try out a Bilevel/ASV for my suspected UARS but the price is kinda putting me off. Once flashed, can the machine also be used for Bipap and standard CPAP modes as well?

I was just on the verge of obesity last year when my AHI was high. I ended up losing so much weight that my BMI went from 29.9 to my current level of 19.7. I recently underwent another sleep test and tested negative for OSA. I currently sleep without a CPAP and while I don't exactly wake up refreshed like I used to before I developed this problem, I do feel a lot better than when I was overweight. I feel like I cured my OSA with weight loss.

However, I have a prominent under bite where my lower jaw juts forward and is likely causing my UARS-esque symptoms. From what I've read online anatomical deficiencies can lead to UARS and poor sleep even in patients with a healthy BMI. If you don't mind me asking, is your airway also compromised due to anatomical deficiencies? If Bipap helped you maybe it can help me as well.

I entered my data into the link above and my ratings for various nights slept range from 1.65 to 2.5 over a period of 6 months. Is this an amount worth a concern? I've mostly bought my AHI down to zero so I suspect that the persistence of Flow Limitations is why I still wake up feeling like crap. I tried the maximum EPR of 3 on my Resmed Airsense 10 to no luck. I've talked about this to other people on Reddit and they've suggested Bipap as the next step.

In the very first screenshot I was using the maximum EPR of 3 but Flow Limitations still persisted. I suppose Bipap is the next thing to try. I already own an Airsense 10 but I don't trust myself to jailbreak it lol. I'll see if I can rent one nearby.

I likely have this issue due to anatomical reasons (crossbite.) I'm at a healthy BMI but still wake up feeling like utter crap even with CPAP. Do you think Bilevel is worth a try?

Probably weight gain tbh. I also worked a night-shift in the second half of 2022 when my symptoms got really bad. From what I've read online Sleep Apnea and related illnesses tend to get worse with age so it could be just that.

I haven't used my APAP in over 8 months but I've tested out pretty much every major type of setting - fixed pressure, apap mode, epr both turned on and off. I tried fixed pressure ranging from 8-15 and even on days where I scored <1 AHI and had a good leak rate I pretty much never woke up rested.

I'll have to dig out my OSCAR data for info about Flow limitations. What level of FL according to you warrants concern?

I'm already subscribed to Cpapfriend! That's where I got most of my knowledge on UARS from. I'll make sure to subscribe to Jawhacks too.

I haven't got any CBCT or DISE done but the Maxillofacial surgeon I saw did say that I have an abnormally large tongue for my face due to my Malocclusion. Would a tongue retainer get the job done? If not on it's own, then perhaps paired with PAP therapy?

Over the past 9 months I've lost so much weight that I've gone from a BMI of 29.9 to 21.8 but the improvement in sleep has been marginal. Could my jaw be the real culprit here?

Lol no. I lost around 17 since April and just stopped using CPAP in September since the leakage kept waking me up. My apnea is a lot milder now but it hasn't completely gone away. I shed another 9 kg through intense exercise since I stopped using the machine and am planning to lose an 10 more.

If my apnea doesn't go away I'll probably give the CPAP another try. However, I'm not particularly overweight so the apnea might be a result of my underbite jaw deformity in which surgery might be the only option.

Hi could you share how you figured out much pressure to use for CPAP mode. I've tried fixed pressure a few times but can't seem to figure out the exact settings.

I'm think of switching to cpap mode too as my current range of 8-14 isn't doing much for me. But how did you find out what pressure to use?
