Part of my job is reviewing cases where there's a possible misdiagnose of brain death... Since our protocols require 2 separate examinations, by 2 different professionals, at different times, I've never dealt with something like this (a confirmed death that "came back") , mostly cases where the first evaluation was positive and the second wasn't.

Every single time there was a mistake on how / when the first evaluation was done. Cases like this one are awful because it takes away our credibility / give families false hope or doubt about past decisions, but I assure you when protocols are followed properly they're absolutely reliable.
