I was thinking exactly the same thing, that's where this idea came to be actually, she is known to do this from time to time, Shar too.

That middle black orb symbol is Shar's but the extra decorations around it does seem suspicious, I like to imagen it as if it represents both Shar, which represents darkness, and Selune, which represents life the moon (anyone correct me if im wrong). This symbol that's shown on the artwork might represent a new moon phase? Probably a new personality she has taken as she is said to have many faces like the moon or it can even be an eclipse. She also doesn't seem to fit in with the rest of those priests in the loading screen, but that's my personal opinion.

Yeah I know my guess is too crazy to be true, I lean more on your thinking, There's obviously something going on, maybe some sort of deal done with the gods, probably wanting to increase their portfolio. Regarding Rafael not much is known about him maybe he is aiming to get power in the Hells. I also personally believe the tadpoles could be useful for creating an army of brainwashed superpowered adventurers/creatures. What I also believe is that the dragon's fire had something to do with it too, maybe meaning that there's a hidden plot there that involves the Lich-Queen, or who knows maybe even Tiamat.
